Election an external expert in the field of psychiatry who will perform the work for the ordering party for substantive support in the area of psychiatry as part of the work of the Addiction Working Group and perform the work for the performance of a seminar dedicated to preventive activities, in particular presenting the effects of the work of the addiction work group. The subject of the order is to provide expert support in the work of a psychiatric expert appointed for the following jdisease unit: addiction to psychoactive substances, including chemicals among young people 15 years of age and students The main task of the selected expert will be to develop, based on the analysis of the work, the assumptions of the health policy program aimed at the prevention of addictions to psychoactive substances, including chemicals among young peopley 15 years of age and students' detailed description of the subject of the order (SOPZ) is Annex No. 1 to the ToR.

I.

In order to demonstrate the absence of grounds for exclusion from the proceedings ordering in the circumstances referred to in art. 24 sec. 1 and art. 24 sec. 5 points 1, 2, 3, 4, 8 of the Public Procurement Law, the Employer requires documents referred to in - 5 items 1-6, 9-10, in connection with - 7 para. 1 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure.

The Contractor shall submit in the form of a single document (Single European Order Document - "EAT") a declaration constitutingpreliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the proceedings and meets the selection criteria - art. 25a of the Public Procurement Law

Amount of the required bid bond: 50.00 PLN (in words: fifty zlotys 00/100).

The Contractor is obliged to pay the bid bond by the end of terminu for submitting offers designated in SIWZ. The Employer allows any form of payment of the bid bond - in accordance with art. 45 par. 6 Public Procurement Law

The Employer envisages the use of the so-called the "return" procedure in accordance with Article 24aa of the Public Procurement Law

the Ordering Party does not provide for supplementary orders referred to in Article 67 paragraph 1 items 6 and 7 and Article 134 (6) (3) of the Act Pzp.

Referring to Section IV.2.6): "The minimum period in which the bidder will be bound by the offer" The Contracting Authority informs that for 2 (three months) 60 days should be accepted.

V.

The Contracting Party declares that being the Data Administrator and having regard to the provisions of the Act of 10.5.2018 on the protection of personal data (hereinafter: RODO) implemented security measures adequate to the risks identified by them and providing for statutory safeguardsego protection of personal data transferred as part of the proceeding. A detailed information clause from art. 13 RODO is contained in the SIWZ.
