Main repair of 6 power units from three 218Md SA134 series railway vehicles

The subject of the order is the main repair of batches of 6 power units from 3 218Md railway vehicles series SA134 with the numbers 026, 027, 028 together with additional works, assembly of repaired power units, commissioningflames of drive units mounted on individual vehicles and a review of P2-1 of 2 engines on each vehicle. Each batch contains two power units.

The subject of the order is the main repair of batches of 6 power units from 3 type 218Md rail vehicles, series SA134, numbers 026, 027, 028 together with additional works, assembly of repaired power units, commissioningdrive units mounted on individual vehicles and a review of P2-1 of 2 engines on each vehicle. Each batch contains two power units.

To confirm that there are no grounds for excluding the contractor from participation in the procedure, pursuant to Art. 24 paragraph 1 point 12 23 (subject to Article 133 paragraph 4 of the Public Procurement Law) and paragraph 5 point 1 of the Public Procurement Law, the contractor must submit on call forreferring:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders in proceedings;

2) the contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or contributions to insuresocial or health care or in the event of such judgment or decision being issued, documents confirming the payment of these debts together with any interest or fines, or the conclusion of a binding agreement on the repayment of those debts;

3) the contractor`s statement that he has not been ordered to prevent him from applying on public procurement;

4) froman entry from the relevant register or the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or in the register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law

the Contractor declares that he has third party liability insurance in the scope of his business activityless for the amount of PLN 1, 000, 000.00 (in words: one million zlotys) and is obliged to maintain insurance in the abovementioned to the extent and to the minimum of this amount throughout the duration of the contract, and provide copies of the policy to the contracting authority.
