Reviewing the fourth maintenance level of P4 complete 37AN and JBG carts from 5 railway vehicles type 218Md SA134 series numbers 015, 016, 017, 018, 019, including wheelsets

In order to confirm the absence of grounds for exclusion of the Contractor from participation in the Proceedings, pursuant to art. 24 sec. 1 item 13-23 (subject to Article 133 (4) of the Public Procurement Law) and para. 5 point 1 of the ActPPL, the Contractor must submit to the ordering party:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders in the Procedure;

2) declaration of the Contractor about the lack of a final court judgment or final administrative decision on resignation from thetaxes, fees or social security or health insurance premiums, or in the event of such a verdict or decision, documents confirming payment of such receivables, including interest or fines, or a binding agreement on repayment of such claims;

3) Contractor's statement about the lack of a decision on the as a preventive measure, a banfor applying for public orders;

4) excerpt from the relevant register or central register and business information, if separate regulations require an entry in the register or in the records, in order to confirm the absence of grounds for exclusion under art. 24 sec. 5 point 1 of the Public Procurement Law Act

the Contractor must prove that: he is insured against civil liability in the scope of the conductedbusiness activity related to the subject of the order for the sum insured not less than PLN 1, 000, 000.00
