Contractors who are not subject to exclusion pursuant to art. 24 paragraph 1 point 12 23 Public Procurement Law and pursuant to art. 24 paragraph 5 item 1 of the Public Procurement Law

To prove noneon the basis of exclusion and fulfillment of the conditions for participation in the procedure, the Awarding Entity requires the submission of:

1) a statement valid as at the date of submission of tenders to the extent indicated in the Annex to the ToR. The information contained in the statement will constitute a preliminary confirmation that the Contractor is not subject to exclusion from the procedure and meets the conditions for participation in the procedure.the form of the uniform European order document (EAT) prepared according to the standard template of the form specified in the implementing regulation of the European Commission;

2) the capital group in accordance with art. 24 paragraph 11 Public Procurement Law, the Contractor, within 3 days from the date of posting information from the opening of offers on the Employer`s website www.gczd.katowice.pl;

3) information from the National Register of Penaltieswithin the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of bids;

4) the Contractor`s statement that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or social security contributions or health or in the case of issuing such an exeye or decision of documents confirming the payment of these debts together with any interest or fines or conclusion of a binding agreement on the repayment of these debts;

5) the Contractor`s statement that he has not been subjected to a preventive measure against him from applying for public contracts.

Issues of submitting documents to Contractors having their registered office or place inflats outside the territory of the Republic of Poland are regulated by - 7 of the Regulation of the Minister of Development of 27.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure (Journal of Laws 2016 item 1126).

To confirm the requirements specified in the Annex from No. 2.1 to No. 2.99 (assortment price specification), the Contractor shall submit an offerĆ:

- for the other packages, the statement: we have implemented and certified in the structures of its activity, the system of quality standards ........ (specify) or / and SEP qualifications (Strategoria E, D) employees performing SIAM inspections and / or training certificates in the field of servicing medical equipment and apparatus issued also by other entities (specify which) * package No. ..........

Contractors who intend to join to the tender, they must make a bid bond before the bid opening date in the amount of: 26, 190 PLN (the bid security for individual parts is given inTerms of Reference). The Purchaser does not allow significant changes to the provisions of the contract in relation to the content of the offer, except for changes based on art. 144 of the Public Procurement Law (amendments in accordance with the provisions in the ToR)
