Subscription service of medical equipment and devices in 2020-2022

Subscription service of medical equipment and apparatus in 2020-2022 under 28 separate items, detailed in the technical specifications of the OP documentation.

Main Site: KOTS - Stara Zagora Ltd., Stara Zagora, Dr. Todor Str.Stojanovic "No. 15 or at the contractor`s service base.

Subscription servicing of sterilization and aspiration equipment, detailed by type and quantity in the specification of medical equipment / equipment for subscription service from technical specifications of the OP documentation.

Participants should be entitled to work with sources of ionizing radiation under the Safe Use Acttitle of nuclear energy (ASUNE) (ie hold a license issued by 31.12.2017 or hold a registration certificate made after 1.1.2018) under the subject matter of the OP for the relevant type of medical equipment - if the tenderer is bidding for lots containing such sources (where applicable) - refers to lots 11, 22, 23, 24, 27 and 28.

AccordingThe nature of the set conditions is proved when submitting a tender with completion of Section A, Part IV of the EDES, and upon signing the OP contract it is proved with a valid certificate of registration for activities with sources of ionizing radiation under Art. 56 para. 3 item 1 of ASUNE for the subject of the OP for the respective type of medical equipment / equipment, performed after 1.1.2018, or a valid license for work with theshutters of ionizing radiation under Art. 58, para. 1, item 3 of ASUNE (in the version of the law until 31.12.2017) under the subject of the OP for the respective type of medical equipment / equipment issued until 31.12.2017 - refers to lots Nos. 11, 22, 23, 24, 27 and 28.

When submitting tenders, they certify the absence of these grounds by filling in the applicable fields of Part III of the OEDOP, and at the conclusion of the contract for the award of the OP - with the following documents:

- under item 1 - with a certificate of criminal record, issuedfrom a district court,

- under item 2 - by the relevant competent authority,

- under item 3 - with a unit from the municipality at the seat of the assignor and with a unit from the municipality at the seat of the participant,

- under item. 4, item 5, item 7, item 8, item 9 and item 10 - with a declaration according to a sample of the OP documentation,

- under item 6 - with a unit of the EA Inspectorate General

The grounds under Article 54, paragraph 1, items 1, 2 and 7 of the Public Procurement Act apply to the personsabout Art. 54 para. 2 and para. 3 of the PPL. Pursuant to Art. 104, para. 2 of the PPA the contracting authority announces that the evaluation of the technical and price proposals will be made before the pre-selection of the participants. For this reason, it is permissible to place the price offer not in a sealed envelope, but to be placed freely in the tender documents. When a participant in the OP is an association that is not a legal entity, he shall representa document showing: the legal basis for its creation; the rights and obligations of the participants in the grouping; the distribution of responsibility among its members; the activities that each member of the association will carry out; appointing a partner to represent the grouping for the purposes of the OP; negotiating joint and several liability between the participants in the merger.
