Performing an annual review and periodic renovation of the Municipal Thermal Waste Treatment Plant in Konin in 2020 and 2021 along with the supply of materials and spare parts

The subject of the contract is the performance of periodic renovation of the Plant Thermal Waste Management in Konin, in 2020 and 2021 inparticularities:

Main Site: Miejski Zakład Gospodarki Odpadami Komunalnych Sp.z o.o. in Konin - ul. Sulańska 13.

The scope of work during the annual inspection is cleaning and carrying out necessary repairs and replacements of worn boiler components and devices, found during operation and during external and internal inspection. A detailed description of the subject of the contract is contained in the ToR (partPart II, point 4, item 4.5.1.)

This condition will be considered fulfilled if the contractor has qualifications in accordance with the Act on technical inspection of December 21, 2000 (Journal of Laws of 2019, item 667) to conducting activities in the scope of:

For part I

1) repair of steam boilers, permanent pressure tanks, non-pressure tanks and low-pressure tanks for toxic or corrosive materialsnon-pressure tanks and low-pressure tanks for flammable liquids, steam pipelines connecting the boiler with a turbogenerator, elements of pressure and non-pressure devices, transmission pipelines, technological pipelines,

2) modernization of mobile platforms, cranes, hoists, winches, and carriage trucks.

For parts II

1) repairs of non-pressure tanksand low-pressure tanks for flammable liquids, elements of pressure and non-pressure equipment, transmission pipelines, technological pipelines

2) modernization of mobile platforms, overhead cranes, hoists, winches, carriage trucks.

For part III

Repair of non-pressure tanks and low-pressure tanks for flammable liquids, steam pipelines combineboiler with a turbogenerator, elements of pressure and non-pressure devices, transmission pipelines, technological pipelines,

On confirmation of compliance with the condition, the Contractor upon request of the contracting authority shall submit valid licenses in accordance with the Technical Inspection Act (Journal of Laws of 2019, item 667) required in the proceedings. for part I, part II and part III

In case of skloading the joint bid, the condition will be considered when one of the contractors jointly applying for the award of the contract has the abovementioned entitlement.

This condition will be met if the contractor is insured against civil liability in the scope of conducted activities related to the subject of the contract for the amount of

110, 000, 000.00 PLN:

- for part I - minimum 50, 000, 000.00 PLN,

- forand Part II - a minimum of PLN 30, 000, 000.00,

- for Part III - a minimum of PLN 30, 000, 000.00.

If offers are submitted for more than one part, the Employer requires that this condition is met for each part, on which the offers are submitted.

In the event of confirmation of compliance with the condition, the Contractor shall, at the request of the contracting authority, submit a document confirming that the contractor is insuringliability from the scope of conducted activity related to the subject of the contract for a guarantee sum not less than that required in the proceedings in respect of Part I, Part II and Part III.

If for a justified reason the contractor cannot submit documents regarding the financial or economic situation required by the contracting authority may submit another document, whichyw sufficiently confirms that the condition of participation in the procedure described by the contracting authority is met. than indicated by the customer.

I. For awardContracts may be applied by Contractors who: are not subject to exclusion pursuant to art. 24 paragraph 1 of the Act and additionally pursuant to art. 24 paragraph 5 point 1. Pursuant to Article 24 paragraph 8 of the Act, the Coll. May perform the "self cleaning" procedure. documents:

1. Information from the National RegisterCriminal within the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submission of tenders.

If a joint bid is submitted, this document must be submitted by all Contractors jointly applying for the contract.

2. Copy from the relevant register or from the central register and information on business activity, if separatethe provisions require an entry in the register or records to confirm the absence of grounds for exclusion under Article 24 paragraph 5 point 1 of the Act. If a joint bid is submitted, this document must be submitted by all Contractors jointly applying for the contract.

3. Statements of the Contractor about the lack of issuing a final court judgment against him or the final administrative decision on arrearswith the payment of taxes, fees or social security or health insurance contributions, or in the event of such a judgment or a decision confirming the payment of these receivables together with any interest or fines, or concluding a binding agreement on the repayment of those receivables.

4. Contractor`s declarations of non-judgment against him as a preventive measureof the prohibition to apply for public contracts.

The information contained in the JEDZ statement constitutes preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. Statements in the scope of section 3 4 are contained in the text EAT.

Note! The Employer requests from the Contractor, which relies on the abilities or situation of other entities on the principles set out in art. 22a ustawy, presentations in relation to these entities of the abovementioned documents

- for part III - 54, 000.00 PLN,

IV. The rules for relying on the capabilities or situations of other entities are set out in the SIWZ part IV points 6.3 - 6.9

V. The Contractor submits together with the JEDZ offer in electronic form, signed with a qualified electronic signature. EATING submission rules are set out in the ToR part V V 7.

VI. The principles of personal data protection are set out in the ToR part XVIII

VII. The contractor within 3 days of posting on the website the information referred to in art. 86 section 5 of the Act, shall provide the Employer with a statement in electronic form regarding belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Act. Along with submitting the declaration, the Contractor may provide evidence that links with another Contractor do not lead to a distortion of competition in the procurement procedure.
