The subject of the order is technical serviceservice, maintenance, repair and maintenance of the technical infrastructure of the facilities of the Voivodship Specialist Hospital named after. Janusz Korczak in Słupsk Sp. z o.o. located in Słupsk at ul. Hubalczykow 1, as well as in Ustka at ul. Adama Mickiewicza 12.

1. The subject of the order is technical and operational service, maintenance, onright

And maintenance of the technical infrastructure of the facilities of the Janusz Korczak Voivodship Specialist Hospital in Słupsk Sp. z o.o. located in Słupsk at ul. Hubalczykow 1 and also in Ustka at ul. Adama Mickiewicza 12 consisting in

in particular:

1) Servicing technical and operational maintenance, maintenance of equipment, installations and technological steam networksand steam boiler room as a backup safeguard for the production of process steam

2) Service and maintenance, maintenance, operation of equipment, installations and networks of the water boiler room, boiler heat exchanger, central heating, process heat and hot water:

3) Service and maintenance, maintenance, continuous maintenance of equipment, installations and cold networkswater and sanitary and rain water drainage

4) Technical and operational service, maintenance, operation of equipment, installations and power networks generating, transmitting and consuming electricity

5) Technical and operational service, maintenance, maintenance of equipment, installation of ventilation systems mechanical, local air conditioning and equipment crefrigeration systems installed in the Employer`s facilities.

6) Technical and operational service, maintenance, maintenance of equipment, installation of refrigeration systems installed in the Employer`s facilities.

7) Technical and operational service, maintenance, repairs in the scope of carpentry, glass, locksmith services, welding, painting, roofing, construction, help

8) Technical and operational service, maintenance within the scope of providing services of maintaining facilities, areas, equipment etc. external

9) Services in the scope of maintaining the safety of facilities use and preventing the occurrence of further damages caused by external factors affecting construction works.

10) Serviceh related to the work carried out at the request of the Ordering Party after prior agreement with the Ordering Party

11) Technical service and maintenance, maintenance, operation of equipment, medical gas installations

12) Provision of the biodecontamination service in hospital organizational cells using the Ordering Party`s VHP 1000ARD device or other if during the validity of the contractThe new Employer shall provide the Contractor;

15) Services in the scope of maintaining the safety of facilities use and preventing damage as a result of external factors affecting the facility;

2 . The detailed scope of requirements is specified in Part IV of the ToRZ Description of the subject of the order and attachments to the description which is an integral part of this specification.

The Employer did not specify the condition in this respect. IDW.

The Employer requires a security on due performance of the contract in these proceedings - in accordance withpoint 19 IDW.

6. Grounds for contractor exclusion

1. The Contractor is excluded from the contract award procedure in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12-23 of the Public Procurement Law

2. At the request of the Employer, the Contractor is obliged to submit the following statements or documents:

1) To confirm compliance with the conditions for participation in the procedure:

a) not applicable

2) In order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings:

a) information from the National Criminal Record in the scope specified in art. 24 paragraph 1pt. 13, 14 and 21 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

b) the Contractor`s statement that he has not issued a final court judgment or final administrative decisionon arrears on arrears with the payment of taxes, fees or social security or health insurance contributions, or in the event of such a judgment or a decision confirming the payment of such receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables;

c) statements of the Contractor of the lack of judgment against him titlepreventive measure of the prohibition to apply for public contracts;

2.1.) If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point 7.6.2 a d IDW:

a) lit. a) submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial authority or adminof the country in which the Contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21. The document should be issued not earlier than 6 months before the deadline for submitting bids.
