1. The subject of the orders is: Performing maintenance services of built-up property located at ul. Piechoty 16 and 17 in Elbląg.

1. The subject of the orders is: Performing maintenance services of built-up property located at ul. Infantry 16 and 17 in Elbląg.

2. A detailed description of the subject of the order is set out in Annex 2 to the ToR.

1. Contractors who are:

1) are not subject to exclusion,

2) they meet the conditions for participation in the proceedings, regarding: a) competences or rightsń to conduct a specific professional activity, if it results from separate regulations,

b) economic or financial situation,

c) technical or professional capacity.

2. The ordering party did not specify a condition regarding competences or entitlements to conduct a specific professional activity.

3. In order to confirm the lack of grounds for exclusion, the Contractor whose offer has been rated the highestshe, will present at the request of the Ordering Party before the order is granted within the prescribed, not shorter than 10 days, the current date as of the day of submitting the following statements and documents: a) information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act and, regarding a conviction for a prison sentence, to the extent specified by the ordering party on the basis ofknows art 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting offers;

b) excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to confirm the lack grounds for exclusion under art. 24 sec. 5 point 1 of the Act;

c) Contractor's statement about bfor the issue of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums, or in the event of such a verdict or decision being issued, documents confirming the payment of such claims together with any interest or fines or the conclusion of a binding agreement in on repayment treceivables - according to the template, constituting Annex 7 to the Terms of Reference;

d) declaration of the Contractor about the lack of a decision against him as a preventive measure for the prohibition of applying for public orders - according to the template constituting Annexe No. 8 to the ToR.

4. The Awarding Entity will exclude from the proceedings the Contractor against whom at least one of the premises specified in art. 24 sec. 1 item 12-23 andArt. 24 sec. 5 point 1 of the Public Procurement Law.

The Ordering Party has not specified the condition.

1. The Contractor is obliged to lodge a security deposit in the amount of PLN 1 100.00 (say: one thousand one hundred zlotys and 00/100) before the deadline for submission of tenders. The Contracting Authority anticipates changes to the provisions of the concluded contract indicated in Part XXVIII of the ToR.

3. The Contractor is obliged to submit on the date specified in Part XX and specify the formin a part of the SIWZ, an offer consisting of: a) a completed offer form with the content in accordance with the template provided in Annex 1 to the SETC;

b) completed price form, according to the template constituting Annexe 1A to the ToR;

Note! The offer form and price form (attachment 1A to the ToR) are related documents. The prices used in both documents should be the samee;

c) a completed EAT document (or EIN documents if applicable in Part XV point 2 in the case of joint application or Part XV point 3 in the case of relying on the potential of a third party);

d) list of works entrusted to Subcontractors as long as the Contractor will carry out the order with the participation of subcontractors according to the template specified in Annex 4 to the Terms of ReferenceZ;

e) power of attorney if it is required (due to authorization to sign the offer or joint application for the order;

f) obligations of the third party referred to in Part XV point 3.3 (or liabilities) as long as the Contractor relies on the potential third party;

g) the original document confirming the payment of the bid bond, provided that the tender deposit is made in a different form than in cash.six documents drawn up in electronic form and signed with a qualified electronic signature must be submitted using the Procurement Platform.

It is unacceptable to submit the Offer in any other way, otherwise being null and void.

4. With reference to Section IV.2.6) "Minimum period in which the bidder will be bound by the offer" The Contracting Authority informs that in 2 (two) months it is necessary to accept 60 days.
