The purpose of the public procurement is the provision of medical devices necessary for treatment by extra-purified treatment. renale for the university hospital of Reims. The public contract is divided into 7 detailed lots in Annex 1 to the CCTP who is in free access on the site https://www.marches-publics.gouv.fr

For a candidate appearing alone or for each member of the group of economic operators, the declarations and information required in the letter of nomination and appointment of thents (DC1 in the version of 26.10.2016 or free form), and in particular:

(a) the presentation of the candidate, alone or in a grouping of undertakings (with the name of the representative, the form of the grouping or, where appropriate, the role of the agent of the joint consortium and the distribution of benefits);

(b) the declaration on the honor of the individual candidate or of each member of the grouping for justensure that it does not enter into any of the bidding cases provided for in Articles 45 and 48 of Order n 2015-899 of 23.7.2015.

For a candidate standing alone or for each member of the grouping of economic operators, the following information and documents are provided:

- a statement of the applicant's overall turnover and, where applicable, the figureof the area of activity covered by the public contract, covering at most the last 3 financial years available according to the date of creation of the company or the beginning of the economic operator's activity, in the the information on these turnover figures is available,

- if, for a justified reason, the economic operator is unable to produce thes information and documents requests concerning the economic and financial capacity, it is authorized to prove its economic and financial capacity by any other means considered appropriate by the university hospital center. The candidate can provide this information and documents via form DC2 in the version of 26.10.2016 or in free form.

Duration of the public market: thePublic contract is due for the period from the date of receipt of the notification until 31.10.2019. This initial period is tacitly renewable for 3 12-month periods, ie: - from 1.11.2019 to 31.10.2020,

- from 1.11.2020 to 31.10.2021,

- from 1.11.2021 to 31.10.2022.

Lot conditional: for lot 6, the public market is linked to a condition (example: expectation of allocation budgetarea, establishment of a new protocol, adoption of a new technique, etc.). The university hospital center will decide whether or not to conclude the corresponding public contracts when choosing the most economically advantageous offers. Production of specimens, samples and associated technical documents: applicants, including outgoing suppliers, will providespecimens, samples and technical documents in accordance with the terms and conditions specified in Article V of the CCTP. The test samples are intended for testing by users. They must meet the conditions for their evaluation under clinical conditions in humans, except for lot 5 and lot 6. For batches 5 and 6, the samples arestines has table tests and can therefore be supplied non-sterile. The absence of submission of specimens, samples and associated technical documents will be grounds for rejecting the candidates' proposal. Specimens and samples must be sent no later than the date and time limit for receipt of tenders. Items provided as specimens and as sampleslons are neither taken back nor invoiced by the candidate.
