The subject matter of the order is the organization of the organization of the AZS AGH Krakow section of competitions in league games in the season 2019/20 - Kc-zp.272-268 / 19:

Organization of the organization of the AZS AGH Krakow section of the competition in the men's volleyball league at the central level in the season 2019 / 20.

1. Contractors, who are not subject to exclusion pursuant to art. 24 sec. 1 item 12-23 and art. 24 sec. 5 points 1 of the Public Procurement Law Act

2. Documents submitted to the ordering party's request:

A / a copy from the relevant register orfrom central records and information on business activities, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion based on Art. 24 sec. 5 point 1 of the Pzp;

B / information from the National Criminal Register, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law, issued no earlier than 6 months before the expiry of the deadlinedeadline for submitting offers;

C / Certificate of the competent head of the tax office confirming that the Contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submitting offers;

D / Certificate of the relevant organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or confirm another documentthat the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting offers;

E / contractor's statement that he has not issued a final court judgment or final administrative decision about his tax arrears, payments or social security or health insurance premiums or - incin the case of issuing such a verdict or decision - documents confirming payment of such receivables with possible interest or fines or conclusion of a binding agreement regarding repayment of such receivables;

F / declaring the contractor about the lack of a ruling against him as a preventive measure for not applying for public orders; G / Contractor's statement about not being abortedpaying taxes and local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716);

H / List of services performed, and in the case of periodic or continuous services Also, during the last 3 years before the deadline for submitting bids, and if the period of business is shorter in this period, along with the specification of their valuebones, objects, dates of completion and entities for which services have been performed, and attaching evidence determining whether these services have been performed or are performed properly. - Evidence confirming that the services have been performed (or are performed) are duly: - a) references or other documents issued by the entity for which the services were performed, and in the case ofor (b) the contractor's statement - if for a justified reason of objective nature the contractor is unable to obtain the documents referred to in point a);

If the contractor makes a statement, he is obliged to state the reasons for the lack of possibility to obtain the evidence referred to in point (a). a)

In the case of recurrent or continuous benefits, the references still to be performedor other documents confirming their proper implementation should be issued no earlier than 3 months before the deadline for submission of tenders - in accordance with Annex No. 4

I / List of persons referred by the contractor for the implementation of the order together with information on their professional qualifications, entitlements, experience and the education necessary to complete the public contract, and this onek-ż scope of activities performed by them and information about the basis for disposing of these persons - according to Annex No. 5 of the ToR.

3. Foreign contractors - If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in point 2 lit. A-D) submits the documents indicated in clause 7.5.1 of the Terms of Reference, respectively. - The Employer does not describe, not youmeans a specific condition in this respect.

The offer must be secured by a bid bond in the amount of:

The tender deposit should be submitted before the deadline for submission of bids.

The bid bond can be paid in one or more of the following forms:

- bank guarantees, or guarantees of the Social Credit and Savings Fund, with the surety that the cash register is always handrail- - bank guarantees, - insurance guarantees - sureties granted by entities referred to in art. 6b par. 5 point 2 of the Act of 9.11.2000 on the establishment of the Polish Agency for Enterprise Development (Journal of Laws of 2018, item 110).

The date of payment of the bid bond in cash is considered the date of crediting the account of the Employer.

2. The Employer demands that the Contractortogether with the offer:

a) Completed and signed by the person authorized to make declarations of will on behalf of the Contractor Bid Form in accordance with Annex No. 1;

b) Uniform European Order Document,

d) Power of Attorney or other document, from which the right to signing the offer and other documents submitted together with the offer, - e) Proof of payment of the bid bond,

f) Commitment or otherokuments confirming the availability of resources by other entities.

3. The Contractor submits the offer in electronic form via the Platform at https://eProPublico.pl/.

4. Form of documents:

a) The offer and statement of EAT, are made, under pain of nullity, in electronic form and are accompanied by a qualified electronic signature.

B) The power of attorney must be submitted in the form ofelectronic document or an electronic copy certified by a qualified signature. c) Documents or statements referred to in the Regulation on the types of documents that the contracting authority may request from the contractor in the procedure for awarding the contract (Journal of Laws of 2016, item. 1126 as amended), are submitted in the original in the form of an electronic document orin an electronic copy of the document or a certified statement as a true copy.

d) The attestation of conformity to the original is made by the contractor, the entity in whose capacity or situation is the contractor, the contractor jointly applying for the public contract or subcontractor, in the field of documents or statements that concern each of them.

e) Certifiede for the conformity to the original of an electronic copy of the document or statement is made using a qualified electronic signature.

5. Within 3 days from the date of placing by the Employer on the website information on the opening of offers referred to in Article 86 par. 5 of the Public Procurement Law, the Contractor is obliged to provide the Employer with a declaration of affiliation or lack of affiliationand to the same capital group that is worth mentioning 24 sec. 1 point 23 of the Public Procurement Law. With the submission of a statement, the Contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure.
