The rental of self-service measurement kiosks by the National Health Fund Headquarters

The subject of the contract is the rental of self-service measurement kiosks by the National Health Fund Headquarters in accordance with Annex 1 to the ToR.

1. Contractors who are not subject to exclusion pursuant to Art. 24 paragraph 1 item 12 23 of the Act.

2. In order to demonstrate the lack ofleave to exclusion from the proceedings referred to above within the time limits specified in Chapter VI, Contractors must submit the following documents:

a. information from the National Criminal Register (...) in accordance with - 5 point 1 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that the Employer may request from the Contractor in the procurement procedure (Journal of Laws of 2016 .Pos. 1126 as amended);

b. the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding late payment of taxes (...) in accordance with - 5 point 5 of the abovementioned Regulation;

c. the Contractor`s declaration of no decision against him as a precautionary measure forbidding the public contract pursuant to - 5 item 6 of the abovementioned Regulation;

D. Contractor`s statement on belonging or not belonging to the same capital group (...) in accordance with - 5 point 10 of the abovementioned Regulation;

e. together with the offer valid as of the day of submitting the EAT offer in electronic version in accordance with chapter VI.

1. The contracting authority informs that in the course of the proceedings, based on art. 24 aa paragraph 1 of the Act, will first evaluate the offers and then examine whether Wykoa person whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings.

2. If the device offered is not a medical device, the offer should be accompanied by certificates or other permits of the competent official quality control institution allowing the use of devices in Poland that meet specific standards or specificationstechnical support. If the device offered is a medical device bearing the CE mark, a current registration application should be attached, i.e. a Declaration of Conformity with details attachments if the medical device offered (model) is not listed directly on the pages of the CE document.

For high-risk medical devices: class II B or class III application for Regof medicinal products, medical devices and biocidal products is required at the stage of putting the product into use, so the customer only requires a declaration of their registration.
