2.1) Thermal film for mammography compatible with Drystar Axys 20 x 25 cm:

2.2) Drystar Axys 25 x 30 cm Compatible Mammographic Thermal Film:

Value of the highest CS 30 000 RON.

5) According to the provisions of Art. 193 of Law 98/2016, the contracting authority will accept in the offers of the DUAE (Unic Achi Document), instead of the documents required to demonstrate qualification requirements, including exercise capacity, as preliminary evidence. The DUAE can be downloaded for completion and submission at https://ec.europa.eu/growth/toolsdatabases/espd/filter (instructions for completing DUAE are available at www.e-licitatie.ro). Prior to assignment-of the Framework Agreement, the contracting authority will ask tenderers ranked in the first three places to submit up-to-date supporting documents demonstrating that all the qualification criteria are met, according to the information contained in the DUAE. DUAE will be submitted, as the case may be, also by the third supporter / subcontractors; 6) Fiscal Attestation Certificate issued by the Administratorthe Financial Documents - the operators will submit these certificates during the evaluation, as up-to-date supporting documents demonstrating the fulfillment of the qualification criteria according to the information contained in the DUAE, and they must certify the lack of overdue debts at the time of their deposit; Fiscal certification. taxable persons and taxable persons-local taxes and other local budget revenues - issued by the City Hall. Operators will submit these certificates during the assessment as up-to-date supporting documents demonstrating that the qualification criteria are met in accordance with the information contained in the DUAE, and they must attest the absence of overdue debts at the time of their submission.the supporters and the subcontractors must not find themselves in the situations provided for in art. 164, 165, 167 of Law no. 98/2016 and art. 60 of Law 98 / 2016.

Note: The certificates required in this section may be presented in any of the following forms: original / legible copy with the reference "according to the original".will complete the DUAE by the economic operators participating in the award procedure with the information related to their situation

The supporting documents proving the fulfillment of the assumed by completing the DUAE are to be presented at the request of the contracting authority by the bidders placed on the first 3 places in the interim ranking prepared to complete bid evaluation.
