Purchase and implementation of the radio communication system encrypted by DMR, case no. 109 / BŁiI / 19 / DG / FBW

The subject of the order is Purchase and implementation of the DMR encrypted radio communication system under the project "Strengthening the fight against crime and terrorism by CBŚP."

Basic (guaranteed) order includes sales and delivery to the Employer's headquarters:

- for Task 1- Handheld DMR radio in the amount of 340 pcs including 132 sets of active hearing protectors and carrying out the instruction. A Contractor who can not be excluded from the proceedings under Art. 24 sec. 1 and 5 of the Public Procurement Law.

In order to demonstrate the lack of grounds for exclusion from the proceedings for awarding the order, the Ordering Party will demand the following documents:

1. excerpt from inclproper register or from central records and information about business activities, if separate regulations require an entry in the register or records, in order to demonstrate the lack of grounds for exclusion under Article. 24 sec. 5 point 1 of the Public Procurement Law - 2. certificate of the competent head of the tax office confirming that the contractor is not in arrears with paying taxes, issued no earlier than 3months before the deadline for submission of tenders or applications for admission to participate, or other document confirming that the contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained the legally required exemption, postponement or distribution for installments of arrears or pre-paymentsRome in its entire implementation of the decision of the competent authority.

3. a certificate of the relevant organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submittingrt or applications for admission to participate in the proceedings, or other document confirming that the contractor has concluded an agreement with the competent authority regarding the repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment of installments due to law or suspension in full implementation of the decision of the competent authority.

4. information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law and, regarding a conviction for a prison sentence, to the extent specified by the ordering party pursuant to art. 24 sec. 5 points 5 and 6 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids or motions for admission to participate in the procedure - 5. street lighadmit the contractor about the lack of a final court judgment or final administrative decision on default of taxes, fees or social or health insurance premiums, or in the event of issuing such a verdict or decision documents confirming the payment of these receivables with any interest or fines or conclusion binding a heaphe is referring to the repayment of these receivables.

6. the contractor's statement about the lack of a ruling against him as a preventive measure for the prohibition of applying for public orders.

7. the contractor's statement about the lack of a final conviction of a conviction court for a restriction of liberty or a fine to the extent specified by the awarding entity pursuant to art. 24 sec. 5 points 5 and 6 of the PPL Act.

8. the contractor's statement that he has not been the subject of a final administrative decision on the breach of his obligations under labor law, environmental law or social security regulations to the extent specified by the awarding entity under art. 24 sec. 5 point 7 of the Public Procurement Law

9. the contractor's statements about not paying taxes and local fees about whichh referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) - the Contractor having its registered office or place of residence outside the territory of the Republic of Poland shall submit documents as described in sec. 6 Chapter VII of the SIWZ.

II. Conditions for participation and required documents are specified in Chapter VI and VII of the ToR.

III. Before signing the contract, the Ordering Party will require the Contractory, whose offer has been selected, to provide security for the proper performance of the contract in the amount of 10% of the gross price of the basic order
