Contractors who can apply for the order:

1. They are not subject to exclusion from the procurement procedure pursuant to art. 24 paragraph 1 point 12) point 23) of the Act, subject to art. 133 section 4 of the Act.

The offer submitted via the purchasing portal must be accompanied by the current as of the day of submitting the offernotification constituting a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure. The said Contractor is obliged to submit in the form of a uniform document (Annex 3 to the ToR), in the form of an electronic document, signed by a qualified electronic signature, in accordance with the principles described in Chapter VII of the ToR.

A) In the case of an offersubmitted by Contractors jointly applying for the award of the contract, the requirement to submit a single document applies to each of the Contractors jointly applying for the award of the contract. A uniform document for each of the Contractors jointly applying for the contract must be submitted, otherwise being null and void, in the form of an electronic document, signed by a qualifiedelectronic signature. These documents confirm the fulfillment of the conditions for participation in the procedure and the lack of grounds for exclusion to the extent that each of the Contractors demonstrates compliance with the conditions for participation in the procedure and the lack of grounds for exclusion.

B) If the Contractor relies on the resources of other entities on the principles set out in art. 22a of the Act, in order to demonstrate the lack of existence against them, pwithdrawal from exclusion from participation in the procedure and compliance, to the extent that the Contractor refers to their resources, the conditions for participation in the procedure, submits uniform documents regarding these entities. A single document for each of the entities on whose abilities or situation the Contractor relies on the principles set out in art. 22a of the Act, must be submitted, under pain of nullity, in fthe electronic document`s signature, signed with a qualified electronic signature. conditions that will be required on the basis of art. 26 section 1 of the Act:

1) information from the National Criminal Record to the extent specifiedin art. 24 paragraph 1 item 13), item 14) and 21) of the Act, subject to art. 133 section 4 of the Act, issued not earlier than 6 months before the deadline for submitting bids;

2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or other supporting document, that the Contractor has entered into an agreement with the competent tax authority on the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or distribution in installments of overdue payments or withholding in full the execution of the decision of the competent authority;

3) certificate of the competent local unit organization of Zakład Ubezpieczeń SpołSocial Insurance Fund or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with the payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the Contractor has concluded an agreement with the competent authority regarding repayment of these receivables along with any interestami or fines, in particular, obtained by law exemption, deferment or installment of overdue payments or withholding in full the decision of the competent authority. website indicated in Section I.3) of the announcement.
