Requirement of fulfillment of the terms and conditions of personal status according to - 32 par. 1 Act no. 343/2015 Coll. on public procurement as amended. (the "Public Procurement Act").

The Applicant proves that the conditions are fulfilled-in part in public procurement under Section 32 or Section 152 of the Public Procurement Act, in accordance with the provisions of the Public Procurement Act. In the case of an applicant who makes a group of contractors involved in a public procurement, proof of compliance with the conditions of the individual's personal status for each member of the group is required. 1 letter.-e) The Procurement Act proves a member of the group only in relation to the part of the subject of the procurement to be ensured. - Dissemination of the adequacy of the use of each specified condition of the part of the public procurement contract in accordance with - 32 of the Public Procurement Act Section 38, par. Article 5 of the Public Procurement Act: The conditions for participation in a person's position will arisefrom - 32 and - 38 par. 1 Act no. 343/2015 Coll. on the award of the public procurement contract: A single European document for the application of the Public Procurement Act is a document by which the economic entity may provisionally replace the documents proving compliance with the terms and conditions of the public limited liability companyravateľom. Documents proving the fulfillment of the conditions of the partially replaced by a single European document shall be submitted to a person according to - 8 of the Public Procurement Act, a successful applicant according to - 39 par. 6 in time and in part, designated by the contracting authority. A specimen of the European Single Document is submitted by the person under Section 8 of the Public Procurement Act in the form of the tender documents. The application must be written in Slovak or Czech. Requests delivered to another address or in another language or incomprehensible applicationwill not respond. Upon receipt of the complete application, I will submit to the interested party the supporting documents. The public procurer / procuring entity / person according to - 8 of the Public Procurement Act advises the interested parties to ask for the tender documents even if they have not obtained the tender documents from the contact person. In the event that the interested party does not request the tender documents, the contracting authority / procuring entity / person pursuant to Article 8 of the Act-the public procurement can not ensure the delivery of explanations, changes and other information that could influence the development of a qualified offer for this tender. In the event that the potential entrant does not ask for the bids of the bids, he / she takes on his / her own responsibility and risk. The tender documents are available to the public and to the profile of the contracting authority / entity / person under-Section 8 of the Public Procurement Act, which is established in accordance with Section 148 3 of the Act on Public Procurement. The public procurer / procuring entity / person under - 8 of the Public Procurement Act will disclose any explanations, changes, additions and responses to accepting a request for correction on his / her profile within the scope of the procurement. Any communication between the contracting authority / procaravateperson / person pursuant to Section 8 of the Act on Public Procurement and Entrepreneurs / Applicants will be exclusively in the Slovak language. The Czech language will be accepted by the enthusiast / candidate.

5. All costs and expenses associated with the preparation and submission of offers, documents proving the fulfillment of the conditions of participation and other documents related to this invitation are the interested person / applicant without financial charge to the publicin the case of a procurer / procurer / person pursuant to Section 8 of the Public Procurement Act, irrespective of the outcome of the public procurement. The originators or certified true copies of the required documents shall be provided by the bidders, unless stated otherwise. All documents must be valid and reflect the actual state at the time when the applicant enters public procurement. Documents demonstrating compliance with the terms of the Participationand applicants with a seat outside the territory of the Slovak Republic must be submitted in a flooded language and must be submitted to the Slovak language in addition to the documents submitted in the Czech language. If there is a difference in the content of the content, the judge is the official translation in the (Slovak) language.

7. If the bidder does not have the power to place a share of the order on behalf of third parties, that fact must be expressly statedis in your offer.

8. The public procurer / procuring entity / person according to - 8 of the Public Procurement Act reserves the right not to accept 1 offer and not to conclude the contract with any tenderer if the prices in the submitted offers will be higher than the estimated value of the order. Tenders may be dispatched by post or personally submitted at the said point of contact in point 1.1 of the-documents, working hours from 08.00 am to 16.00.
