The service of making a box promoting the musical creativity of Polish composers of the last century

The subject matter of this order is the box service promoting the musical creativity of Polish composers of the last century as part of project "100 out of 100. Musical decades of freedom" in twolanguage versions: Polish and English.

The subject of this order is the box service promoting the musical creativity of Polish composers of the last century as part of the project "100 per 100. Music decades of freedom" in two language versions: Polish and English.

On the occasion of pthe November 100 anniversary of the independence of the Polish State, the Employer (PWM), together with the cultural institutions invited to cooperate in Poland and abroad, is preparing a prestigious musical project called "100 out of 100 Musical Decades of Freedom." The main goal of its implementation is creating an unprecedented opportunity to familiarize with a wide range of recipients, -both in Poland and around the world, with a representative attempt at musical music, the pen of Polish composers of the last century

I. The ordering party does not set specific requirements in this respect - II.

1. The Contractor submits an offer to participate in the proceedings via the Form for submitting, changing, withdrawing the offer or application available on ePUAP and also made available to the miniPortalu. The public key necessary to encode the offer by the Contractor is available to contractors on the miniPortal. In the offer form, the Contractor is obliged to provide the ePUAP mailbox address, on which correspondence related to the proceedings will be kept

2. The offer should be made in the Polish language, keeping the electronic form, in particular in the data format .doc, .docx andsigned with a qualified electronic signature. The method of submitting the offer, including the encryption of the offer, is described in the Regulations for the use of miniPortal. The offer must be submitted in the original. The ordering party does not allow the submission of a scan of the offer with a qualified electronic signature.

3. The offer must be accompanied by the Single European Order Document in the form of an electronicthe qualified electronic signature, and then, together with the files constituting the offer, compress to a single archive file (ZIP).

4. The Contractor may, before the deadline for submission of tenders, change or withdraw the offer via the Form to submit, change, withdraw the offer or application available on ePUAP and also made available on the miniPortal. The method of changing and withdrawing the offer will beas described in the User's Manual available on the miniPortal.

The Contractor after the deadline for submission of offers can not effectively change or withdraw the offer.

The Ordering Party does not set specific requirements in this respect. 1. A Contractor who meets the condition of participation in the procedure and can not be excluded from the proceedings under Art. 24-paragraph. 1 point 12-23 and art. 24 sec. 5 points 1 Public Procurement Law.

2. In order to confirm compliance with the conditions for participation in the proceedings, the contractor will be required to submit the following declarations and documents:

3.2. an excerpt from the relevant register or from central records and information abouteconomic activity, if separate regulations require an entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 points 1 of the Public Procurement Law

3.3. information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13 or 14 Public Procurement Law, issued no earlier than 6 months before the deadline for submitting offers,

3.4. information from countriesego Register of Criminal Records in the scope specified in art. 24 sec. 1 point 21 Public Procurement Law (collective entities), issued not earlier than 6 months before the deadline for submitting bids,

3.5. the contractor's statement that he has not been the subject of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums-or in the event of issuing such a verdict or decision documents confirming the payment of these claims together with any interest or fines or conclusion of a binding agreement regarding the repayment of such debts and a declaration of the contractor about the lack of a ruling against him as a preventive measure for not applying for public orders.

3.6. statements of the contractor about prdependence or lack of belonging to the same capital group; in the case of belonging to the same capital group, the contractor may submit together with the declaration documents or information confirming that connections with another contractor do not lead to distortion of competition in the procedure - II. Refers to section IV point 2.6 of this announcement: The Contractor is bound by the offer until the expiry of the terminu: 60 days.

III. The Contracting Authority informs that in the present proceedings it provides for the possibility of carrying out the procedure referred to in art. 24aa of the Public Procurement Law, ie the Awarding Entity may first evaluate the offers and then examine whether the offer of the Economic Operator whose offer has been evaluated as the most advantageous is not subject to exclusion and fulfills the conditions for participation in the procedure.sharing orders referred to in art. 67 section 1 point 6 and 7 of the PPL - IV. The offer must be secured by a bid bond in the amount of PLN 12, 000 (twelve thousand PLN 00/100). The form, date and place of payment of the bid bond, as well as its return and retention have been specified in the ToR.
