Providing printing services for NBP in the field of offset printing and bookbinding work. - Reference Number: SEZ / DKRZ-WPO-RS-241-0121 / DA / 18 - The subject of the orders is the provision for 36 months of printing services by the Contractor for the Employer in the field of offset printing and bookbinding.

Main SiteWarsaw: The subject of the order is the provision of printing services by the Contractor for the period of 36 months for the Employer in the field of offset printing and bookbinding, including in the field of books, brochures, leaflets, folders, posters, bands, labels and other prints, and delivering ready products to the headquarters of the Employer.

Information about the subject of the order and its-are included in the assortment and price form constituting Annex 4 to the ToR and in the model contract constituting Annex No. 13 to the ToR.

1. The ordering party sets a bid bond in the amount of PLN 12, 000.00 (in words: twelve thousand PLN 00/100). Detailed information on the bid bond is included in the ToR.

2. The Contractor is bound by the offer for a period of 60 days

3. The offer should be accompanied by:

a) Actuual Single European Order Document. Information on the preparation of EED is included in the SIWZ:

b) Assortment form, completed and signed in accordance with the specimen specified in the SIWZ;

c) Three copies of the leaflet, made by the Contractor using the offset technique in accordance with the requirements specified in the SIWZ;

d) One sheet of printing from the offset machine, to indicate the implementation of the leafletsin point c) in offset printing.

4. The Contracting Authority provides for the application of the procedure of procedure referred to in art. 24aa of the Public Procurement Law

5. Statements and documents for demonstrating fulfillment of conditions for participation in the proceeding and no grounds for exclusion The Contractor is obliged to submit in a form consistent with the provisions of - 14 of the Regulation of the Minister of Development of 26.7.2016 on-types of documents that the Employer may request from the Contractor in the proceedings for the order.

6. The ordering party will exclude the contractor from the proceedings in the circumstances indicated in art. 24 sec. 1 and item 5 point 1, 2, 4 and 8 of the Public Procurement Law

7. A list of statements or documents confirming the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion, which the Purchaser requests onart pond 26 of Public Procurement Law: -1) In order to demonstrate compliance by the Contractor with the conditions of participation in the procedure, the contracting authority requests the documents specified in section III.1.3,

2) To demonstrate the absence of grounds for exclusion, the contracting authority requests: a) Documents and declarations referred to in -5 points 1-6, 9 above regulation,

b) Documents or statements referred to in - 7 and 8 above regulation - if the contractor has-registered office or place of residence outside the territory of the Republic of Poland or if the person whose document relates is residing outside the territory of the Republic of Poland, c) The statement referred to in -5 point 10 above Regulation ... equipment.

8. Detailed information on how to prepare and submit offers are specified in the ToR.

9. Detailed information on statements and a documento made by the Contractor to confirm the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion - they contain the ToR and the provisions of the abovementioned Regulation.
