Printing and delivery of magazines and books for the Book Institute

The subject of the order is printing and delivery of magazines and books for the Book Institute

The subject of the order is printing, binding, packaging, packaging, transport and preparation for distribution of magazines for the Book Institute in Krakow.

The scope of the contract includes usthe following publishing houses:

1) "Dialog" monthly,

2) "Literatura na Świecie" bimonthly,

3) "Nowe Książki" monthly,

4) "Tworność" monthly,

5) "Teatr" monthly,

6) Kwartalnik "Puppet Theater".

7) Books from the series "In the circle of Parisian CULTURE"

All indicated magazines have the ISSN number and the books have the ISBN number.

1. Contractors who are not subject to the order may apply for:exclusion pursuant to art. 24 paragraph 1 and art. 24 paragraph 5 points 1 and 8 of the PPL Act; 2) meet the conditions for participation in the proceedings.

2. Each Contractor must enclose with the offer a current statement as at the date of submitting the bids in the scope indicated in Annex 2 to the ToR. The declaration is made on the form of the Single European Contract Document (EAT), prepared in accordance with the standard modelform specified in the European Commission implementing regulation issued on the basis of art. 59 clause 2 of Directive 2014/24 / EU, hereinafter "the statement". The information contained in the statement will constitute a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure.

The contractor should download the electronic version from the contracting authority`s websiteeditable EAT.

3. In the case of joint application for the Contract by the Contractors, the declaration referred to in 2 shall be made by each of the Contractors jointly applying for the contract. This statement is to confirm that the conditions for participation in the procedure have been met, no grounds for exclusion to the extent that each of the Contractors shows that they have met the conditions for participation in the procedure, no grounds for exclusion.ia.

4. The Contractor, which refers to the resources of other entities, in order to demonstrate the lack of existence of grounds for exclusion and fulfillment in the scope in which it refers to their resources, the conditions of participation in the proceedings also submits the statement referred to in point 2 regarding these entities.

5 . Statements and documents to be submitted by the Contractor, whose offer will be onrated above, or the Contractor, whom the Employer will call for submission of all or some statements or documents confirming that they meet the conditions for participation in the procedure and there are no grounds for exclusion in the cases referred to in art. 26 section 2f Pzp, are presented in Section VI.3

6. The documents included in the offer may be presented only in the formoriginals in the form of electronic documents or electronically certified by the Contractor for compliance with the original copy. Statements, including those made on the basis of templates constituting attachments to the ToR, should be submitted in the original and bear a qualified electronic signature. Compliance with the original of all pages of copies of documents included in the offer must be confirmedby a person (or persons if several persons are authorized to represent the Contractor) signing (signing) the offer in accordance with the content of the document specifying the Contractor`s legal status or the power of attorney attached to the offer.

7. The rules regarding the certification of copies of documents or statements for compliance with the original are set out in the ToR.

8. In the event of submission by the Contractors todocuments containing data expressed in currencies other than PLN, the Purchaser, as the conversion rate of the currency in which the value was estimated, will adopt the average exchange rate of the National Bank of Poland (NBP) as at the date of publication of the contract notice in the EU Official Journal.

9. Assessment of compliance with the conditions for participation in the procedure will be made according to the formula: "meets does not meet".

10. Ordering Party, part. 24aa Pzp, provides for the possibility to first evaluate the offers, and then examine whether the Contractor whose tender has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the procedure.

11. The contractor is obliged to pay a deposit in the amount of: PLN 5, 000.

12. Information about documents that the Contractor should submit if he complies withrespecting the conditions for participation in the proceedings, it invokes the resources of other entities pursuant to art. 22a of the Public Procurement Law are in Section III.1.3

The Employer does not specify in this respect any requirements that the Contractor is required to demonstrate in a special way

Statements and documents that the Contractor whose bid will be highest rated or Contractor will be required to submitThe contracting authority will call for submission of all or some statements or documents confirming that they meet the conditions for participation in the procedure and there are no grounds for exclusion in the cases referred to in art. 26 section 2f of the PPL Act:

1. In order to confirm that there are no grounds for excluding the Contractor from participation in the procedure, the Awarding Entity requests:

1) a certificate from the competent treasury officethe confirmation confirming that the Contractor is not in arrears with payment of taxes, issued not earlier than 3 months before the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority on the repayment of these receivables along with any interest or fines, in particular obtained exemption, deferral or installment provided for by lawy overdue payments or withholding in full of the decision of the competent authority;

2) a certificate from the Social Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions issued not earlier than 3 months before deadline for submitting bids or other document pconfirming that the Contractor has entered into an agreement with the competent authority on the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferment or installment of overdue payments as provided for by law or withholding in full the execution of the decision of the competent authority;

3) the Contractor`s statement on failure to issue a final court judgment against him or herthe final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions or in the event of such a judgment or decision on documents confirming the payment of these claims or the conclusion of a binding agreement on the payment of these claims;

4) information from the National Criminal Register in the scope specified in art. 24 paragraph. 1 item 13-14 of the Public Procurement Law and issued not earlier than 6 months before the deadline for submission of bids;

5) information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

6) an excerpt from the relevant register or from the Central Register and Information on Economic Activity, ifseparate provisions require an entry in the register or records to confirm the absence of grounds for exclusion pursuant to Article 24 (1). 5 point 1 of the PPL Act. If the register is publicly available, this document is not required (for example, the Register of Entrepreneurs of the National Court Register, CEIDG);

7) the Contractor`s statement that there was no decision against him as a precautionary measureon public procurement;

2. The Employer requires that in a situation where the offer of the Contractor who, by demonstrating compliance with the conditions for participation in the procedure, relies on the resources of other entities on the principles set out in Art. 22a of the Public Procurement Law, was considered the most favorable, the Contractor presented in relation to these entities the documents listed in points from 1.1 to 1.7, confirming the lack of grounds for exclusionteaching from public procurement proceedings.

3. The Contractor within 3 days from the date of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law, will provide the Ordering Party with a declaration of belonging or not belonging to the same capital group within the meaning of the Act of 16.2.2007 on competition and consumer protection (Journal of Laws of 2019, item 369, as amended.), referred to in art. 24 paragraph 1 point 23 of the PPL Act. Along with submitting the declaration, the Contractor may provide evidence that links with another Contractor do not lead to a distortion of competition in the procurement procedure.
