The subject of the order is printing and delivery to the recipients of the monthly "Labor Inspector" and inserts to the monthly "Labor Inspector" issued by the National Labor Inspectorate Chief Labor Inspectorate. The publishing house has ISSN number 0239 3417. A detailed description of the subject of the order is included in Annexes 2 and 2a to the Specification of the Essential Terms of the Order.

1. The customer requestssubmitting together with the offer:

1) a single European order document (EAT) statement,

2) power of attorney if applicable,

3) obligations of another entity if the Contractor relies on the capabilities or situation of another entity (Article 22a of the Public Procurement Law (hereinafter PPL) )),

4) document confirming the payment of a bid security in the forms specified in art. 45 item 6 Pzp.

2. Documents submitted byd awarding the order, at the request of the Employer:

1) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids,

2) an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or register, to confirm the absence of grounds for exclusion under Article 24 paragraph 5 point 1 Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids,

3) certificate from the competent tax office head confirming that the contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or another document confirming thatthe contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables, together with any interest or fines, in particular, he obtained the exemption, deferral or distribution of installments in arrears of outstanding payments or withholding in full the execution of the decision of the competent authority,

4) certificate of the appropriate local organizational unit Zakład Ubezpieczeń Społecznych or Kasa Rolniczego Ubezpieczenia Społecznego (Agricultural Social Insurance Fund or Kasa Rolniczego Ubezpieczenia Społecznego) or other document confirming that the contractor is not in arrears with payment of social security or health insurance contributions issued not earlier than 3 months before the expiry of the time limit for the submission of tenders, or another document confirming that the contractor has entered into an agreement with the competent authority on repayment of these receivables together with any interestand / or fines, in particular, he obtained the exemption, deferment or installment of overdue payments in installments provided for by law, or withholding in full the execution of the decision of the competent authority,

5) the Contractor`s declaration of non-payment of local taxes and fees, referred to in the Act of 12.1. 1991 on local taxes and fees (i.e. Journal of Laws of 2018, item 1445, as amended),

6) onproviding the Contractor with a failure to issue a final court judgment or a final administrative decision regarding late payment of taxes, fees or social security or health insurance contributions, or in the event of issuing such a judgment or decision, documents confirming payment of these claims together with any interest or fines, or concluding binding antceases to repay these debts,

7) the Contractor`s statement that no decision has been issued against him as a precautionary measure forbidding public procurement,

8) list of services rendered, references, list of technical devices, list of persons

3. Within 3 days from the date of posting on the website the information referred to in art. 86 section 5 Public Procurement Law The contractor is obliged to do evila declaration of belonging or not belonging to a capital group, as referred to in art. 24 paragraph 1 point 23 of the PPL Act

4. If the Contractor relies on the resources of other entities on the principles set out in art. 22a of the Public Procurement Law, will be obliged to provide the Employer with documents and statements of those entities referred to in section III.1.1 in point 1 point 1 and 2 and point 2 point 1 7announcement.

1. The Employer will exclude from the proceedings the Contractor:

1) who has not demonstrated that the conditions for participation in the proceedings referred to in section III.1.3 of the notice have been met,

2) for which the conditions specified in art. 24 paragraph 1 point 13 23 and in art. 24 paragraph 5 points 1 and 8 of the Act.

2. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of:

1) informsz o. specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law;

2) an excerpt fromof the register, tax office and ZUS or KRUS certificates submit a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming accordingly that:

a) he is not in arrears with taxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding the repayment of these debts together withinterest payments or fines, in particular, he obtained the exemption, deferral or installment of outstanding payments as provided for by law, or withholding in full the execution of the decision of the competent authority,

b) its liquidation has not been opened or bankruptcy has been declared.

3. Documents referred to in item 2 point 1 and point 2 point b should be issued not earlier than 6 months before the expiry of the terminu make offers. The document referred to in item 2 item 2 letter a should be issued not earlier than 3 months before the deadline for submission of tenders.

4. If in the country in which the Contractor has its registered office or place of residence or place of residence there is a person to whom the document relates, the documents referred to in item 2 do not appear, they shall be replaced by a document containing appropriate referencenotification of the Contractor, indicating the person or persons authorized to represent him, or a statement of the person to whom the document was to be submitted, submitted before a notary public or before a judicial, administrative or professional or economic self-government body competent for the seat or place of residence of the Contractor or the place of residence of this persons, with due datesas referred to in points 3.

5. In case of doubt as to the content of the document submitted by the Contractor, the Employer may contact the competent authorities of the country in which the Contractor has its registered office or place of residence or place of residence, respectively, of the person to whom the document relates, to provide the necessary information regarding this document.

6 . Contractor based in the territory of Rof the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the information from the KRK relates, submits the document referred to in point 2 point 1, to the extent specified in art. 24 paragraph 1 points 14 and 21 of the PPL Act. If in the country in which the person to whom the document was supposed to reside has no place of residence such documents are not issued, then gon a document containing the person`s declaration made before a notary public or before a judicial, administrative or professional or economic self-government body competent for the place of residence of that person. Such a document should be issued not earlier than 6 months before the offer submission deadline.

7. The contracting authority requires a deposit of PLN 2, 000 beforethe bid submission deadline. The bid bond in cash should be paid by bank transfer to the PIP GIP bank account located at the NBP District Branch in Warsaw with the number 16101010100083541391200000

8. The bid bond paid in a form other than cash provided for in art. 45 item 6 of the Public Procurement Law, the Contractor submits in the original, in electronic form with a qualified electronic signature of the document issuerentu.
