Z oo, divided into 16 parts according to the types (varieties) of reducers.

I. You can apply for the order if you:

1. not subject to exclusion;

2. meet the following conditions for participating in the procedure:

2.1. have the competence or authority to conduct specific professional activitieshey, if it results from separate provisions of the Order. it does not specify this condition

2.2. are in an economic or financial situation. Zam. it does not specify this condition

2.3. have specific technical or professional capacity as specified in point III.1.3 of the notice.

II. From the procurement procedure Zamaw. excludes Wyk. on the basis of the conditions specified in art. 24 section 1 item 12, 13 a c, 14 (subject to item 7.1.3 of the ToR) 23 of the Public Procurement Law (hereinafter the Public Procurement Law) and in art. 24 paragraph 5 point 1 8 Public Procurement Law, taking into account art. 133 section 4 Public Procurement Law and art. 24 paragraph 7 12 Pzp.

III. Assessment of compliance with the abovementioned conditions will be made in accordance with the formula "meets does not meet", based on information contained in JEDZ and the documents to be provided by the University in order to confirm the fulfillment of the conditions of participation in the Progress and the lack of grounds for exclusion progressiveFor preliminary confirmation of compliance with the condition of participation in the procedure set out in points III.1.1 and III.1.3 of the notice, it will be sufficient for the Author to submit a proper statement to JEDZ in Part IV: Qualification criteria, section α.

IV. List of declarations and documents to be provided by the Contractors in order to confirm compliance with the conditions for participation in the procedure and the absence of grounds for exclusionia.

Together with the offer, which can be made on the Offer Form constituting Annex 1 to the ToR, the Contractor shall submit:

1. current statement as at the date of submitting bids to the extent indicated in these ToR in the form of a single document;

2. justification of the trade secret, if the Contractor reserves the trade secret in accordance with point 12.9. TOR;

3. appropriate powers of attorney;

4. inin the event that the Contractor relies on the capacities or situation of other entities, obliging these entities to provide him with the necessary resources at the disposal of the contract;

5. document confirming the payment of the bid security, if it is paid in non-monetary form (details regarding the bid security are explained in item 10 of the ToR);

6. declaration in accordance with art. 138c paragraph 1 point 4 of the Actof the specimen constituting Annex 12 to the ToR.

I. Zam. before awarding the order, he will call the top-rated bid, to be submitted within a designated period, not shorter than 10 days, valid at the date of submitting statements or documents confirming the circumstances referred to in art. 25 paragraph 1 Pzp, it is:

1. documents referred to in point III.1.1 and point III.1.3 of this notice (referred to in point 8.2 of the ToR);

2. information from the KRK to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the PPL and regarding convictsand for an offense against imprisonment, to the extent specified by Zam. based on Article. 24 paragraph 5 points 5 and 6 of the PPL, issued not earlier than 6 months before the deadline for submitting bids;

3. zaśw. the competent head of the Tax Office confirming that the bill is not in arrears with taxes, issued not earlier than 3 months before the deadline for submitting bids, or another document confirminghim that the Wyk concluded an agreement with the competent tax authority on the repayment of these receivables together with any interest or fines, in particular he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority;

4. zaśw. the relevant field organizational unit of ZUS or KRUS or another documentu confirming that the Wyk is not in arrears with the payment of social security or health insurance contributions issued no earlier than 3 months before the deadline for submitting bids, or other document confirming that the Wyk has entered into an agreement with the competent authority regarding the repayment of these receivables, together with any interest or fines, in particular, he was granted exempted by lawe, deferment or installment of overdue payments or withholding in full the implementation of the decision of the competent authority;

5. an excerpt from the relevant register or from CEiDG if separate provisions require an entry in the register or the register in order to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 Pzp;

6. tolerated aspect. Execution of the failure to issue a final court judgment or final judgment against himadministrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the event of such a judgment or decision - documents confirming the payment of these receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables;

7. tolerated aspect. Execution of the title against himpreventive measure prohibiting the use of public contracts;

8. declarations of performance regarding the failure to issue a final judgment of the convicting court for the offense of restriction of liberty or a fine to the extent specified by Zam. based on Article. 24 paragraph 5 points 5 and 6 of the Pzp;

9. tolerated aspect. exec. on the lack of issuing to him the final administrative decision on violation of obligations arising fromand labor law, environmental protection law or social security provisions to the extent specified by the Employer pursuant to art. 24 paragraph 5 point 7 Pzp;

10. tolerated aspect. Execution of non-payment of taxes and local fees, referred to in the Act on local taxes and fees.

II. Zam. informs that the personal data provided will be processed in accordance with the information contained in "Obligationat the information point "available on the Order website at www.psgaz.pl, in the tab: For the contractor / Information obligation. 86 item 5 of the Public Procurement Law, provides the Order with a declaration of belonging or not belonging to the same capital group.

IV. The Ordering Party informs that it will conductseriously in accordance with art. 24aa Pzp.
