Conditions for participation of applicants in public procurement related to personal status pursuant to - 32 of the Public Procurement Act

1.1. Information and formalities necessary to assess the fulfillment of the terms and conditions: Applicants must comply with the conditions of part time under Article 32 1 of the Public Procurement Act

1.1.1. Uchadzač pfulfills the conditions of the part concerning the personal status according to -32 par. 2, resp. ods. 4, 5 by submitting original documents or certified true copies of the documents, pursuant to Section 152 of the Public Procurement Act

1.1.2. Entrepreneur entered on the list of economic entities pursuant to the regulations effective until 17 April 2016 fulfillment of the conditions of part time pursuant to - 32 par. (a) in the case of histhe organizational structure of the member / members of the supervisory body and / or the procurator / procurator shall be shown as follows:

1.1.2.1. according to -32 par. (a) by submitting an extract from the criminal record not exceeding three months; or

1.1.2.2. pursuant to -152 of the Act on Public Procurement (after the change of the data in the list of economic subjects and the addition of this information to the list of economic entities pursuant to Article 156 of the Acton Public Procurement)

1.1.3. Agents entered on the list of economic entities pursuant to the regulations effective until 17 April 2016 fulfillment of the conditions of participation in accordance with Article 32 of the Decree. b) letter. c) show the following:

1.1.3.1. according to -32 par. b) letter. c) no more than 3 months of presentation of the health insurance and social insurance certificate and the local tax authority's certificates not older than3 months, or

1.1.3.2. pursuant to -152 of the Act on Public Procurement (after the change of the data in the list of economic entities and the addition of this information to the list of economic entities pursuant to Section 156 of the Public Procurement Act)

1.1.4. An entrepreneurial entity with the status of a legal person to exercise a personal status according to - 32 par. 1 letter. a) the public procurement law is afterit is also possible to submit a copy of the criminal record for the person who is issued by the General Prosecutor's Office of the Slovak Republic no more than 3 months.- The economic entity entered in the list of the public sector partners may prove to meet the aforementioned conditions in the following way: a) by submitting an original document - penalties of a legal person not older than 3 months in the process of public procurementor (b) adding this information to the list of public sector partners by means of a form of notification of the change of data, the design of which the Public Procurement Office (hereinafter referred to as "the Office") publishes on its web site with the attached extract from the register of penalties of a legal person no more than 3 months.

1.1.5. In the case of an applicant who creates a group of suppliersin the public procurement, proof of compliance with the conditions of participation in respect of each individual member of the group is required. The fulfillment of the condition of part-time according to - 32 par. 1 letter. e) the law on public procurement proves a member of the group only in relation to that part of the subject of the order, which I can provide.

1.1.6. If the applicant has a place of business, business or usual residence outside the land Slthe Republic of Poland and the statute of its officers, places of business or usual residence does not issue any of the documents mentioned in - 32 par. (2) of the Public Procurement Act, nor does it issue or provide equivalent documents, it may be replaced by an affidavit in accordance with the rules in force in the State of his office, place of business or usual residence. Terms of participation of tenderers in public procurement concerning financial matterseconomic status and evidence of their proof according to - 33 of the Public Procurement Act

2.1. The bidder shall submit the following original document (s) or his / her certified true copy of which shall prove his / her financial and economic standing: 2.1.1. 2.1.3 Dissatisfaction of the proportionality of the conditions of the part in relation to the subject of the bid: evidence of the financial stability of the applicants, the state of their economic and financial situation, their ability to pay and-to the bank. The fulfillment of the terms and conditions attached to the assessment of the economic and financial standing of the tenderers to such a reasonable extent creates the premise that the performance of the contract will be executed only by such a contractual partner ensuring the smooth fulfillment of the contractual charter by minimizing the risk of non-fulfillment of contractual obligations due to economic instability.to prove the financial and economic status to use the financial resources of another person, regardless of their legal relationship. In this case, the tenderer must demonstrate to the public procurement agent that he will actually use the resources of a person in the performance of the framework agreement in order to prove his financial and economic status. The fact of the second sentence proves that the applicant has a written contractu closed with a person whose sources are meant to prove their financial and economic standing. From a written contract, the person's handbag must appear to provide performance during the duration of the contractual relationship. The person whose resources are to be used to prove his financial and economic status must demonstrate compliance with the terms of part-time status, except for Section 32 1 letter. e) the law ofpublic procurement shall not be subject to exclusionary evidence pursuant to - 40 par. 6 sheets. (a) to (h) and 7. If the applicant fails to furnish the document to the contracting authority with objective evidence to prove the financial and economic status of the document, the financial and economic status may be demonstrated by the submission of another document considered by the contracting authority-suitable.

1. The tender documents ("SP") for the public tender are published from the day of the award of this public procurement contract as established by the electronic tender on the website of the Public Procurement Office www.uvo.gov.sk. The public procurer will also publish in his / her profile any other documents necessary for the preparation of the offer (eg poscapturing the explanation under - 48 of the Public Procurement Act). The contractor shall provide the purchaser with a written request electronically or by post to the address given in Section I, point I.1., Of this notice. Such a request must obviously be the same as that(identification of the order according to the name and of this notice), identification data of the solicitor (business name or names, address, place of business or usual residence of the applicant), contact details of the telephone number and e-mail address). 4. The contracting authority shall provide an explanation of the information required-for the preparation of the offer, a proposal for proving the fulfillment of the terms and conditions in compliance with - 48 of the Public Procurement Act and according to the details stated in the tender documents. The public contracting authority shall exclude from the public procurement of the tenderer, within the scope of evaluation of the conditions under Article 40 of the Public Procurement Act, if, on the basis of reliable information, I am guilty of the suspicion that the applicant has closed-in the given public procurement contract with another economic entity an agreement disrupting the economic tender. Within the scope of this public tender, the provisions of - 66 par. 7 of the Act on Public Procurement, evaluation of the fulfillment of the conditions of participation pursuant to Section 40 of the Public Procurement Act shall be carried out after evaluation of the tender pursuant to Section 53 of the Public Procurement Act. In the case of communication, the Procurer of Procedureis in line with - 187 par. 8 of the Act. in electronic form means communication made by fax or e-mail. The public contracting authority does not enter into a framework agreement with an applicant who has the obligation to register in the public sector partners register under Act No. 315/2016 on the register of public sector partners and on the amendment of some persons for(hereinafter referred to as the Public Sector Registers Act) and are not entered in the register of public sector partners or subcontractors who are required to register with the public sector partners and are not registered in the public sector register.
