Supply of optical microscope, adapted for working with polarized light, equipped with digital camera, computer and specialized softwareq 1 The participant must have achieved a minimum turnover in the scope of the public procurement, calculated on the basis of annual turnover, for the last 3 (three) years from the date of submission of the tender. The participant should provide information in Part IV, Section B, "Economic and Financial Situation" by EEDOP. The selection criterion is evidenced by one or several of the documents.o art. 62, para. 1, item 3 and item 4 of the PPL annual financial statements or their components when required; reference to the turnover in the sphere falling within the scope of the order or another document in the sense of Art. 62, para. 2 of the Public Procurement Act. The guarantee for the performance of the contract is 5% (five per cent) of its value and is presented in one of the following forms: cash, bank guaranteeor insurance covering performance by covering the contractor's liability. The participant designated as a contractor shall choose the form of the performance guarantee itself. The participants in the procedure shall present the documents in sealed, opaque and inscribed packaging in accordance with the requirements of Art. 47, para. 2 of the RIPP. The packaging, according to the requirements of Art. 47, para. 3 of the PPPP, contains documentsunder Art. 39, para. 2 and para. 3, item 1 of the PPPP, an inventory of the submitted documents and a separate, sealed, opaque envelope with the inscription "Suggested price parameters" containing the price proposal under Article 39, paragraph 3, item 2 of the PPPP. shall remove from participation in the procedure a participant for whom one of the circumstances under Article 54, paragraph 1, Article 55, paragraph 1, item 1, item 4 and item 5, article 101, par.. 11 and Art. 107 of the Public Procurement Act and under Art. 3, item 8, without the exceptions under Art. 4 of the Economic and Financial Relations Act with companies registered in preferential tax jurisdictions, the persons controlled by them and their actual owners, Art. 69 of the Law on Counteracting Corruption and for Removal of Unlawfully Acquired Property and Art. 13 para. 1 of the Labor Codemigration and labor mobility as well as a participant who in any way has included somewhere in his offer outside the Envelope "Offered Price Parameters" items related to the price offered and / or parts thereof as well as a participant who in the price its proposal has indicated a total value, exclusive of VAT, for the execution of the contract, which exceeds the total estimated value given in section II.1.5)The Contracting Authority will remove from participation in the tendering procedure whose tender offer does not meet all the specifications of the technical specification. All templates attached to this documentation are binding on the participants and the assignor will remove from participation in the procedure the participant who does not complete the samples according to the instructions of the assignor.
4. Where a participant in a procedure is an association which is not a legal person, a copy of the document shall be required showing the legal basis for the formation of the grouping and the rights and obligations of the members of the grouping; the division of responsibility between the members of the grouping; the activities that each member of the group will carry out in relation to the present public harvest
5. The lack of grounds for removal under Art. 107, Item 4 of the Public Procurement Act, Art. 3, item 8 of the Economic and Financial Relations Act with the companies registered in preferential tax jurisdictions, the persons controlled by them and their actual owners, Art. 54, para. 1 point 1 of the Public Procurement Act other than those under Part III, Section A "Grounds relating to criminal convictions" by EEDOP, as well as Article 69-of the Law on Counteracting Corruption and Forfeiture of Unlawfully Acquired Property are declared in the EEDOP, Part III, Section D "Specific national grounds for exclusion", such as: - if the above mentioned grounds for removal are not available to the participant, the participant notes "No" in the right-hand column, - if the above mentioned reasons are available for the participant(6) In accordance with Article 67 (4) of the Public Procurement Act (PPA), in conjunction with - 29, para. 4 of the Public Procurement Act (PPL), the participant shall state the "Yes" answer in the right-hand column and fill in the next line regarding rehabilitation measures taken. 5 (a) of the Transitional and Final Provisions of the PPA in force from 1 April 2018. The Single European Procurement Document (EEDOP) is mandatory to be submitted electronicallyid. The EEDOP should be provided by the participants in electronic form in accordance with the instructions in section 6, section VIII "Templates of documents and instructions for their preparation" in the tender documentation.
