Delivery of devices performing the role of Mobile Worn Terminal and MDM license framework contract

The subject of the order is the delivery of mobile wearable MDM licenses under the framework agreement. the order is delivery of devices performing the role of Mobile Worn Terminal and the license it holdsby the Employer of MDM software in the following number: MTN set - 1 362 pcs .; quadruple desktop charger - 250 items; single desktop charger - 362 pcs; Famom MDM license for software owned by the Ordering Party - 1 362 pcs. The detailed description of the subject of the order is included in Attachment 2 to the ToR.

The Contractor may apply for the order, which:

Fulfills the following conditionsu regarding the economic and financial situation, including: - Holding financial assets or creditworthiness in the amount not less than: PLN 4, 000, 000.00 (four million zlotys) - for task no. 1 - The contractor who does not apply for the order may apply subject to exclusion from the proceedings based on art. 24 sec. 1 and 5 of the Public Procurement Law. In order to demonstrate the lack of grounds for exclusion from the proceedings onOrdering, ordering:

1. excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to demonstrate the absence of grounds for exclusion pursuant to art. 24 sec. 5 point 1 of the Act;

2. a certificate from the competent head of the tax office confirming that the Contractor is not in good standingtax, issued not earlier than 3 months before the deadline for submitting bids or applications for admission to participate in the proceedings, or any other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables with possible interest or fines, in particular obtained the legally required exemption, deferral ofb spreading overdue payments into installments or suspending the execution of the decision of the competent authority in full;

3. a certificate issued by the relevant organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than3 months before the deadline for submitting bids or applications for admission to participate in the procedure, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding repayment of such receivables, including any interest or fines, in particular obtained the legally required exemption, postponement or payment of overdue payments or interruptionse in full implementation of the decision of the competent authority;

4. information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Act and, regarding a conviction for a prison sentence, to the extent specified by the Employer pursuant to art. 24 sec. 5 points 5 and 6 of the Act, issued not earlier than 6 months before the deadline for submitting tenders or applications for admissionto participate in the proceedings;

5. the Contractor's statement that he has not been the subject of a final court judgment or final administrative decision on late payment of taxes, fees or social security or health insurance premiums or in the event of such a verdict or decision confirming the payment of such claims together with any interest orb fines or a binding agreement on the repayment of these receivables;

6. the Contractor's statement about the lack of a ruling against him as a preventive measure of the prohibition against applying for public orders;

7. the Contractor's statement on the non-delivery of a final conviction of a conviction court for violation of liberty or a fine to the extent specified by the Awarding Entity on the basis ofawie art 24 sec. 5 points 5 and 6 of the Act;

8. the Contractor's statement that he did not issue a final administrative decision to him on the breach of his obligations under labor law, environmental law or social security regulations to the extent specified by the Awarding Entity pursuant to Art. 24 sec. 5 point 7 of the Act;

9. Contractor's statement about not paying for the supplyand local fees, referred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2016, item 716) - the Contractor having its registered office or place of residence outside the territory of the Republic of Poland shall submit documents in accordance with the description contained in paragraph 6 Chapter VII of the ToR.
