1. The subject of the order is:" Delivery and assembly of centrifuges for dewatering of sewage sludge ", in the design formula build as part of the project:" Extension of sanitary sewage system in Radomsko. "

1 The subject of the order is: "Delivery and assembly of centrifuges for sludge dewatering

Sewage", in the design formulabuild as part of the project entitled "Expansion of the sanitary sewage system in Radomsko".

2 The aim of the task is to equip the existing sewage sludge station in the Sewage Treatment Plant in Radomsko at Spacerowa 120 with a technological installation including a decanter centrifuge with accompanying devices to the extent necessary for proper functioning of the installation with the parameters set in ZaNo. 6 to SIWZ. - Description of the sewage treatment plant site and sewage treatment technology applied and treatment of sediments is included in Appendix No. 1 to the ToR.

As part of the adaptation of the facility to new equipment and related requirements, it is planned to replace internal installations: ventilation and electrical lighting and switchgears, renovation of the floor.

It is also necessary to arrangea new power cable for the building of a sludge dewatering station with a cross-section adjusted to the increased power and the telecommunication duct and the communication network.

3. The detailed scope of the order is specified in Part III of the ToR.

4. Detailed description of the subject of the order, scope of the robot and requirements, technical specifications of the performance and acceptance of the construction works in the scope of: - general requirements, - robot rocollective and dismantling works, construction and construction works, - technological installations, - ventilation installations, - electrical installations, - and information on safety and health protection as well as safety and health plan are included in the attached Functional and Functional Program "Delivery and assembly of the dewatering centrifuge sewage sludge "Attachments No. 2-12 to the ToR.

5The adder connected to the SIWZ constitutes an auxiliary material to describe the subject of the order.

The Ordering Party does not specify any requirements in this regard.

The contracting party considers the condition fulfilled if the contractor demonstrates that: - has own funds or creditworthiness of not less than 500, 000, 00 PLN,

- has liability insurance in the scope of conducted activityrelated to the subject of the order for the amount of PLN 300, 000.00.

1. The contractor is obliged to submit an offer consisting of:

1) completed offer form,

2) power of attorney to represent all contractors jointly applying for the order (in case of joint bid),

3) liabilities of the entity referred to in point 5.3, 5.4 and 5.5 SIWZ, if it performsthe essence relies on the abilities of other entities to confirm compliance with the conditions for participation in the proceedings.

2. In order to demonstrate the absence of grounds for exclusion from the contract award procedure and confirmation of compliance with the conditions for participation in the proceedings in the form of a Single European Order Document.

3. The contracting authority does not provide for the award of contracts referred to in Article 67 par. 1 point 6 of the Public Procurement Law

4. The Employer allows the possibility of communication via e-mail address: e-mail: pois2@pgk-radomsko.pl. within the scope specified in Chapter 8 of the Terms of Reference (General Information), point 1, second indent.

5. The Awarding Entity does not provide for the reimbursement of costs of participation in the proceedings, including reimbursement of costs incurred for the purchase of a qualified electronic signature. The offer must besecured bid bond in the amount of: PLN 25, 000.00 (say: twenty five thousand zlotys, 00/100). The bid bond must be submitted before the deadline for submission of tenders and brought in the form specified in Chapter 9 of the ToR.

7. According to art. 24aa paragraph 1 of the Public Procurement Law, the orderer will first evaluate the offers, and then examine whether the contractor whose offer has been evaluated as the most advantageous, is not subject toexclusion and meets the conditions for participation in public procurement proceedings. If the contractor referred to above avoids concluding the contract or fails to provide the required performance bond, the contracting authority may investigate whether it is not subject to exclusion and whether the contractor who submitted the tender highest rated in the remaining offers meets the conditions for participation in the proceedings. . ZamPrior to the order being awarded, the contractor will call the contractor whose offer has been rated the most, to submit within the prescribed, not shorter than 10 days, the current date as of the day of submitting statements and documents confirming fulfillment of the conditions for participation in the proceedings and lack of grounds for exclusion in accordance with 7.4. Terms of Reference.

9. The Contractor may confirm the fulfillment of the conditions for participation in the progresswhere appropriate and in relation to a specific contract, or a part thereof, rely on the technical or professional capacity or financial or economic situation of other entities, regardless of the legal nature of the legal relationship they have with it. The contractor, which relies on the abilities or situation of other entities, must prove to the orderer that by implementingthe order will have the necessary resources of these entities, in particular presenting the commitment of these entities to give him the necessary resources for the needs of the order. This document (eg commitment) should be attached to the offer.

11. The contracting authority will assess whether technical or professional capacities or their financial situation are made available to economic operators by other entitiesor the economic operator allows the contractor to demonstrate compliance with the conditions for participation in the procedure and will examine whether there is any ground for exclusion referred to in art. 24 sec. 1 of the Public Procurement Law and paragraph 6.2. Terms of Reference.

12. The ordering party requests from the contractor whose offer has been selected as the most advantageous, to bring security on due performance of the contract in the amount of 5% of the totalreferred to in the offer.

13. The collateral serves to cover claims for non-performance or improper performance of the contract and must be brought before the contract is signed. Documents confirming the lodging of the security should be submitted before signing the contract with the Ordering Party.

14. The ordering party does not make a reservation on the personal performance of the key part of the order referred to by the contractorin art 36a paragraph 2 Public Procurement Laws
