Contractor`s statements regarding the conditions for exclusion from the procedure, compliance with the conditions for participation in the procedure, in the form of a uniform European order document Annex 3; information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 uPublic Procurement Law, issued not earlier than 6 months before the deadline for submission of bids, the Contractor`s statement that he has not issued a final court judgment to him or a final administrative decision on late payment of taxes, fees or social security or health insurance contributions, or - in the case of such a judgment or decisions - documents confirming payment, youreceivables together with any interest or fines or conclusion of a binding agreement on the repayment of these receivables - Annex 6, the Contractor`s statement that he has not issued a preventive measure against him from applying for public procurement - Annex 7; the Contractor`s statement on belonging or not belonging to the same capital group, Annex 5;an excerpt from the relevant register or from the central register and information on business activities, if separate provisions require an entry in the register or register; valid authorization to operate a pharmaceutical warehouse or customs and consignment warehouse for medicinal products issued by the Main Pharmaceutical Inspector or an equivalent document.

Pursuant to art. 13 (1) and (2) of the RegulationOrder of the European Parliament and of the Council (EU) 2016/679 of 27.4.2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (General Data Protection Regulation) (OJ EU L 119, 4.5.2016, p. 1), hereinafter referred to as "GDPR", I inform you that:

a) the administrator of the Contractor`s personal data is the CenterPrzedsiębiorczości CP Sp. z o.o. running the Poviat Hospital in Pszczyna, ul. Antesa 11, 43-200 Pszczyna, e-mail: zamowienia.szpital@powiat.pszczyna.pl;

b) a personal data protection inspector at Centrum Przedsiębiorczości CP Sp. z o.o. running the Poviat Hospital in Pszczyna ul. Antesa 11, 43-200 Pszczyna is: contact:

d) the recipients of the Contractor`s personal data will be persons or entities to whom the documentation of the procedure based on art. 8 and art. 96 section 3 settingsy of 29.1.2004 Public Procurement Law (Journal of Laws of 2018, item 1986, as amended), hereinafter referred to as "PPL Act";

e) the Contractor`s personal data will be stored in accordance with Article 97 (1) Public Procurement Law, for a period of 4 years from the date of termination of the contract award procedure, and if the duration of the contract exceeds 4 years, the storage period covers the entire duration of the contract;

f) the obligation to provide bythe adviser of personal data directly concerning natural persons is a statutory requirement specified in the provisions of the Public Procurement Law Act related to participation in a public procurement procedure; the consequences of not providing specific data result from the Public Procurement Law;

g) the Contractor has:

- pursuant to art. 15 GDPR, the right to access personal data of individuals regarding

- pursuant to art. 16 RODO`s right to rectify the Contractor`s personal data - exercising the right to rectification may not result in a change in the outcome of the procedure

for the award of a public contract or a change in the provisions of the contract to the extent not complying with the Public Procurement Law and may not violate the integrity of the protocol and its annexes,

- pursuant to art. 18 GDPR, the right to request the administrator to limit data processingpersonal, subject to the cases referred to in art. 18 clause 2 GDPR - the right to limit processing does not apply to storage, to ensure the use of legal protection measures or to protect the rights of another natural or legal person, or due to important reasons of public interest of the European Union or a Member State,

- right to bringand a complaint to the President of the Office for Personal Data Protection, if the Contractor considers that the processing of personal data of natural persons concerning violates the provisions of the GDPR.
