In the above circumstances, the contracting authority willdid not require jun documents;

1. Other documents to be submitted for the offer:

1. INThe completed Offer Form, the required content of the form is contained in Annex 1 to the ToR.

2. Completed Offer cost estimate of the subject of the contract, offered by the contractor, providing the value and information specified in Annex 1 to the draft contract.

3. Permission to operate a pharmaceutical warehouse or a consignment warehouse or a customs warehouse as regards the possession of qualificationsspecific activity or activity if the law imposes an obligation to have it.

4. Document confirming the payment of a bid security as per the requirements described in chapter VIII of this ToR.

5. Current statement constituting a preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings in the form of the Single European Contract Document fromimportant hereinafter EAT

6. Contractor`s declaration of belonging or not belonging to the same capital group according to Annex 4 to the ToR within 3 days of posting on the website the information referred to in art. 86 section 5 of the PPL Act. In the case of belonging to the same capital group, the contractor may submit documents or information along with the declarationconfirming that links with another contractor do not lead to a distortion of competition in the proceedings.
