The subject of the order is the delivery of medicines for the Silesian Center for Rheumatology, Rehabilitation and Prevention of Disabilities. gen. J. Ziętka w Ustroniu Sp. z o.o. in quantities and assortmentnot indicated in Annex 1 to the ToR

The shelf life of delivered medicines cannot be shorter than 6 months from the date of delivery and should be clearly displayed on the packaging.

Offered producty must be registered in the Register of Medicinal Products.

Deliveries of medicines will be carried out:

A / successively on the basis of orders placed by the Employer,

B / within 2 business days of placing the order,

C / between 8:00 am and 8:00 am 12: 00,

D / directly to the Employer`s Pharmacy warehouse.

The packaging of the drugs delivered will show the authorization number allowing them totrading in the territory of the Republic of Poland, in accordance with the requirements in force in this regard. The contracting authority allows the delivery of packaged products in other sizes of packaging, after prior arrangement with the ordering person.

A detailed description of the subject of the order is included in Annex 1 to the ToRAnnex assortment and quantity.

The method of order fulfillment is set out in Annex 2 to the ToR, specimen contract.
