The subject of the order is the delivery of medicinal products, medical devices and disinfectants, divided into 58 packsetow, according to Annex No. 2 to the Terms of Reference - Price form with a detailed description of the subject of the order.

The subject of the order is the delivery of medicinal products, medical devices and disinfectants, divided into 58 packages, in accordance with Annex 2 to the Terms of Reference - Form-price with a detailed description of the subject of the order - license or authorization of the Main Pharmaceutical Inspector in the scope of running a pharmaceutical wholesaler, if the Contractor runs a pharmaceutical wholesaler, - authorization of the Main Pharmaceutical Inspector for the manufacture of medicinal products, if the Contractor is a manufacturer. financial or creditworthinessthis amount:

1) PLN 17 000.00 with the total value of the lower or equal bid submitted: PLN 75, 000.00,

2) PLN 25, 000.00 with the total value of the offer submitted: PLN 75, 000.01 PLN 150, 000.00 PLN,

3) PLN 50, 000.00 at the total value of the offer: PLN 150, 000, 01 PLN 300, 000.00,

PLN 4) PLN 100, 000.00 at the total value of the offer: PLN 300, 000.00 PLN 500, 000 PLN,

5) 200, 000.00 PLN prthe total value of the offer submitted: 500, 000, 01 PLN 750, 000.00 PLN,

6) 300, 000.00 PLN with the total value of the offer submitted: 750, 000, 01 PLN 1, 500, 000.00 PLN,

7) 500, 000.00 PLN for the total value of the bid over PLN 1, 500, 000.00.

The condition for participation in the proceedings is the contribution of the Contractor to the total amount of PLN 244, 791.00. Value of the bid bond for individual umwas placed in the SIWZ in point 20.

In addition, the Contracting Authority requests:

- information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Act and regarding a conviction for a prison sentence, to the extent specified by the ordering party under art. 24 sec. 5 points 5 and 6 of the Act,

- a certificate from the competent head of the tax office confirming that the contractor is not in arrearspayment of taxes or other document confirming that the contractor has concluded an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in full of the decision of the competent authority, - a proper certificatethe local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the contractor is not in arrears with paying social security or health insurance premiums, or other document confirming that the contractor has concluded an agreement with the competent authority regarding repayment of such receivables with possible interest or grin particular, obtained the legally required exemption, postponement or installment payment of overdue payments or suspension in full of the decision of the competent authority, - a copy from the relevant register or from central records and business information, if separate provisions require entry in the register or records to confirm the absence of grounds for foreclosure onArt. 24 sec. 5 point 1 of the Act,

- Commitments on the Contractor being put at the disposal of the necessary resources for the needs of the order. ANNEX NO. 6

- The Contracting Authority demands from the contractor, which relies on the ability or situation of other entities under the terms of art. 22a of the Act, to present documents referred to in items 15.9.3-6 for these entities, - to present the documreferred to in items 15.9.3-7 concerning subcontractors, which he intends to entrust the execution of a part of the order, and which is not an entity on whose capacity or situation the contractor relies on the principles set out in Article. 22a of the Act,

- Contractor's statement on having the characteristics of the medicinal product / safety data sheet, registration certificates, certificates and other documents requiredOrders that are subject to trade in the territory of the Republic of Poland, in accordance with the requirements set out in: Pharmaceutical Law, the Act on Medical Devices Biofuish Law, along with executive acts applicable to the application of the provisions of these Acts, as well as documents confirming compliance with the spectrum for medium activities disinfectants for the medical area and oundertaking to present them at the request of the Employer, within 3 days from the date of such request in accordance with ANNEX 5 to the ToR, - the Employer in questionable situations, during the examination of tenders, to verify the conformity of the proposed products with the description of the subject of the order

In Annex 2 to the ToR, he may ask the Contractor to submit a probek offered products [for packages No. 46-58]. The samples are returned on the application and cost of the supplier. - completed offer form, in accordance with ANNEX 1 to the ToR, - price form with a detailed description of the subject of the order in accordance with ANNEX 2 to the ToR,

- a statement in the form of an EJ in the range indicated by the Ordering Party in the order notice or in the relevant specificationorder conditions. EAT should be sent in electronic form with a qualified electronic signature.
