The subject of the order is the supply of medicines and medical devices in the assortment specified in Annex 1 to the ToR. Detailed conditions regarding the subject of the order are included in Annex 1 to SIWZ.

The subject of the order is the Supply of drugs and medical devices in the range specified in Annex 1 to the ToR. Detailed conditions regarding the subject of the order are included in Annex 1 to the ToR. 24 aa Contractors who are not subject to exclusion may apply for the order. No basis for useluczeniao referred to in art. 24 paragraph 1 and art. 24 paragraph 5 of the Public Procurement Law will be verified on the basis of the uniform European order document (hereinafter: EAT) submitted with the offer, with the content consistent with the template constituting Annex 3 to the ToR. The Contractor, within 3 days of posting on the website the information referred to in art. 86 section 5 of the PPL Act is forwarded by the Employeremu a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act according to of the model set out in chapter II point 4 of the ToR. Along with submitting the declaration, the Contractor may provide evidence that links with another Contractor do not lead to a distortion of competition in the procurement procedure. 1. To confirm complianceconditions of competence or authorization to conduct a specific professional activity, provided that it results from separate provisions. The Contractor together with the offer will submit a completed EHIP print with the content in accordance with the template constituting Annex 3 to the ToR. 2. Farm Law. and we undertake to make them available on every request of the Customer. with content in line with the specimen in Chapter II, point 5 of the ToR; 2The Contractor`s notification that the medicine offered is reimbursed in accordance with an administrative decision issued pursuant to art. 11 of the Act of May 12, 2011 on drug reimbursement, foodstuffs special live purposes, with content corresponding to the specimen in Chapter II, point 5 of the ToR; 3.Chpl containing the necessary information 4.Ośw that the offered range has documents required by Polish law, onwhich may be placed on the market and used in health care centers in the Republic of Poland with content consistent with the model set out in Chapter II point 5a of the Terms of Reference; 5.
