Successive supply of computer equipment and accessories with a 36-month warranty service

1. The subject of the order is the gradual delivery of computer equipment and accessories together with a 36-month warranty service.

2. The subject of the order under part 1 includes the delivery and warranty service of: PC3 Inżynierski 4 monitor 19 pcs.

3. The detailed scope and description of the subject of the contract is included in the ToR.

Contractors who are not subject to exclusion from the contract award procedure may apply for a public contract.public service pursuant to art. 24 paragraph 1 point 13 point 23 and art. 24 paragraph 5 points 1, 2, 4 and 8 of the Public Procurement Law Act (referred to as the Act announcement), with the proviso that the Contractor is not subject to exclusion in the case referred to in art. 24 paragraph 1 point 13 lit. d of the Act, and in the case referred to in art. 24 paragraph 1 point 14 of the Act, if the person referred to in this provision was pconvicted of a crime listed in art. 24 paragraph 1 point 13 lit. d of the Act.

In the case of Contractors jointly applying for the award of the contract, the conditions described above must be met by each of the Contractors separately.

1. Contractors who meet the conditions of participation in the proceedings regarding economic or financial situation, i.e. they have the ability to apply, may apply for a public contract.retirement funds or funds on the bank account:

1) under part 1 for an amount not less than PLN 50, 000.00 (say: fifty thousand zlotys 00/100);

2) under part 2 for an amount not less than 1, 000 PLN 000.00 (in words: one million zlotys).

2. In the event of applying for the contract under more than one part of the contract, to confirm compliance with thisof this condition, it will be sufficient to demonstrate that the Contractor has financial means or creditworthiness in an amount not less than the quota limit specified for a given part, for which the Contractor applies. In the event that the Contractor attaches documents containing amounts expressed in currencies other than PLN to confirm compliance with the conditions for participation in the procedure, he will orderhe will convert them into PLN. The average NBP exchange rate for the date of publication of the contract notice in the Official Journal of the European Union will be used for the conversion. In the absence of publication of the NBP exchange rates as at the day referred to above, the exchange rate last announced before the above shall apply. as per - 8 item 5 of Resolution No. 51/2002 of the National Bank`s Management Boardu Polish of September 23, 2002 on the method of calculating and announcing current exchange rates of foreign currencies (consolidated text, i.e. from June 8, 2017 (Official Journal of the National Bank of Poland from 2017 item 15) as amended).

3. Assessment of compliance with the conditions required from Contractors will be made according to the formula "0 1", where the value 0 means "does not meet" and the value 1 "meets", based on the statements contained in the uniformon the European Order Document EAT (Annex 2 to the ToR) attached to the offer submitted in the present procedure.

4. In the case of Contractors jointly applying for the award, the condition described above may be met by the group of Contractors jointly applying for the award.

I. The offer must be made in Polish in electronic formqualified electronic signature.

II. To confirm that the offered equipment meets the requirements specified in the Description of the subject of the order (Annex 1 to the ToR), the offer should also include the completed range of parameters of the offered equipment and signed Annex 1 to the ToR, Description of the subject of the contract, as well as the printout from the page: https: //www.passmark.com/products/pt.htm (within the requirementsregarding the processor indicated in Annex 1 to the ToR Description of the subject of the order), not older than 3/12/2019.

III. To confirm compliance with the conditions for participation and no grounds for exclusion. Zam. I will request a copy of which the offer has been rated the highest, to be submitted within a period of not less than 10 days, valid as at the date of submission:

1) info. from the National Reg. Criminal to the extent specified inArt. 24 paragraph 1 item 13, item 14 and 21 of the Act, with the proviso that the Contractor is not subject to exclusion in the case referred to in art. 24 paragraph 1 point 13 lit. d of the Act, and in the case referred to in art. 24 paragraph 1 item 14 of the Act, if the person referred to in this provision has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. d of the Act, issued not earlier than 6 months before its expirym the deadline for submitting bids;

2) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, or other document confirming that the Contractor has concluded an agreement with the competent tax authority on the repayment of these receivables together with any interest or fines, regardinghe has been provided with an exemption, deferral or installment of overdue payments as provided for by law, or withholding in full the execution of the decision of the competent authority;

3) certificate of the appropriate local organizational unit of ZUS or KRUS or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 monthsby the deadline for submission of bids, or other document confirming that the Contractor has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of outstanding payments or withholding in full the decision competent authority;

4) excerpt from the relevant registrystru or from the central register and information on economic activity, if separate provisions require an entry in the register or register (Article 24 (5) point 1 of the Act);

5) statement Contractor about the lack of issuing a final court judgment against him or a final administrative decision on late payment of taxes, fees or social security or health insurance contributions, or in the event ofissuing such a judgment or a decision of documents confirming the payment of these debts together with any interest or fines or concluding a binding agreement on the repayment of these debts (Article 24 paragraph 1 point 15 of the Act) App. 3 to the ToR;

6) statement Contractor on the lack of a decision against him as a preventive measure prohibiting him from applying for the award of the contract (art. 24 of usvol. 1 item 22 of the Act) 4 to the Terms of Reference;

7) the Contractor`s declaration of non-payment of taxes and local fees, referred to in the Act of 12.1.1991, on local taxes and fees App. 5 to the Terms of Reference;

8) the Contractor`s statement on belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Act. 6 to SIWZ.
