1. The subject of the order is delivery new devices for the needs of the Technical School Complex in Koło.

The Awarding Entity does not specify a condition in this respect. referred to in art. 24 paragraph 1 points 12-23 and art. 24 paragraph 5 point 1 Public Procurement Law (chapter VI, section 1 of the ToR)The Employer requires the submission of the following statements and documents:
1) Single European Contract Document (EAT) Annex 9 to the ToR (document (s) together with the offer), completed in the absence of grounds for excluding the Contractor from the contract award procedure based on the circumstances referred to in art. 24 paragraph 1 points 12-23 and art. 24 paragraph 5 point 1 Pzp part IIIsection A, B, C and D EAT;
2) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submission of tenders (document (s) submitted at the request of the Employer by the Contractor whose tender was highest rated);
3) an excerpt from the relevant register or from the central records and information about activitiesbusiness, if separate provisions require an entry in the register or records to confirm the absence of grounds for exclusion under Article 24 paragraph 5 points 1 Public Procurement Law

4) the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding himarrears with the payment of taxes, fees or social security or health insurance contributions or in the event of the issuance of such a judgment or a decision of documents confirming the payment of such receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables Annex No. 7 to the ToR (document / y submitted at the request of the Employerby the Contractor whose bid was highest rated;) top rated);
6) the Contractor`s statement about belonging or not belonging to thesej of the capital group itself (Annex 4 and 5 to the ToR).
