Conclusion of framework agreements for conducting accreditation reviews in hospitals by medical check-ups in the period from the date of conclusion of the contract until December 31, 2020. Wheneverthe term "accreditation visit" or "accreditation review" should be understood as a review of the entity providing health services in terms of meeting the accreditation standards, defined in the Act on accreditation in health care dated November 6, 2008 (Journal of Laws of June, 2013, p. 1) and in the Ordinance of the Minister of Health of 31.8.2009 on the procedure for assessing compliance with pby an entity providing health services accreditation standards and the amount of fees for its implementation (Journal of Laws from 2009, No. 150, item 1216). The purpose of the framework agreement is to determine the terms and conditions of the contracts that may be awarded during the term of the framework agreement (ie from the date of its conclusion to December 31, 2020), in particular prices for a single day of the accreditation visit.Hospitals located in Poland - Conclusion of framework agreements for conducting accreditation reviews in hospitals by medical check-ups in the period from the date of conclusion of the contract until December 31, 2020. Whenever we refer to an "accreditation visit" or "accreditation review", this should be understood as an overview the entity providing health services in terms of fulfilling the conditionaccreditation bodies specified in the Act on accreditation in health care dated November 6, 2008 (Journal of Laws of June, 2013, p. 1) and in the Ordinance of the Minister of Health of 31.8.2009 on the procedure for assessing compliance by the entity providing health services with accreditation standards and the amount of fees for its performance (Journal of Laws from 2009, No. 150, item 1216). The purpose of the framework contract is USconditions for contracts that may be granted during the period of validity of the framework agreement (ie from the date of its conclusion to 31.12.2020), in particular prices for a single day of the accreditation visit. The contracting authority provides for a maximum of 90 accreditation reviews to be carried out under a framework contract by a medical inspector, in whichmore than one visitation of the doctor. The given number is an estimated value, and the actual number of reviews carried out will result from the Employer's current needs. Each review will last a maximum of 3 days. A detailed description of the subject of the contract being the subject of the contract of enforcement is specified in Annex No. 1 to the ToR.

Note: If the contractor will submit an offermercury, the election of which would lead to the creation of a tax obligation in accordance with the provisions on the tax on goods and services, the orderer will add to the price of the tax on goods and services listed therein, which he would be obliged to settle in accordance with these provisions. The Contractor, by submitting the offer, informs the customer whether the selection of the offer will lead to the creation of aordering the tax obligation, indicating the name (type) of the good or service whose delivery or performance will lead to its creation, and indicating their value without the tax amount. The lack of a statement in this respect will be treated as making a statement on the absence of such an obligation. Any negative consequences of this assumption will rest with the contractor. order thec. reserves the right to deduct from the indicated price / remuneration of the tax contractor and premiums, which according to separate regulations is obliged to pay the Ordering Party (applies, among others, to physical persons not conducting business activity). ATTENTION: an offer not submitted using a dedicated form: "Form for submission, modification, withdrawal of the offer or application" will not be considered.it is allowed to submit an offer using, for example, a "communication form." The ordering party will call contractors after the tender and offer evaluation process, the offers will be rated top-down within 10 days, valid as of the day of submitting the declarations and documents confirming the circumstances referred to in points 5.2 and 5.3 of the ToR.
