1. The ordering party does not allow variant offers

2. The ordering party does not provide for settlement in foreign currencies.

3. Awardingwhich does not provide for an electronic auction.

4. The Contracting Authority does not provide for the reimbursement of costs of participation in the proceedings.

5. The ordering party does not anticipate the requirements referred to in art. 29 para. 3a and 29 para. 4 Public Procurement Laws pertaining to employment of persons

6. The ordering party does not reserve the obligation of the contractor to perform the key parts of the order personally.

7. The ordering party does not allow the possibility of submitting an offer in-the form of electronic catalogs or the inclusion of electronic catalogs to the offer.

8. The contracting party conducted a technical dialogue prior to the initiation of the present proceedings.

9. The Contracting Authority does not provide for the conclusion of a framework agreement and does not establish a dynamic purchasing system.

10. The ordering party acting pursuant to art. 24 aa of the Public Procurement Law, will first evaluate the offers and then examine whether the Contractor, -which offer has been assessed as the most advantageous, it is not subject to exclusion and meets the conditions for participation in the proceedings.

11. Contractors who are not subject to exclusion from the proceedings for the award of a contract under Art. 24 sec. 1 point 12-23 of the PPL and para. 5 points 1-3 and 8 of the Public Procurement Law

12. At the request of the Employer, the Contractor is obliged tosubmitting the following declarations or documents confirming no grounds for exclusion:

1) current statement referred to in section III.1.3) sub-point 1 of the Announcement

2) current information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13), 14) and 21) of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids.

3) current certificatethe competent head of the tax office confirming that the Contractor is not in arrears with payment of tax.

4) valid certificate of the relevant local organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document.

5) excerpt from the relevant register or from the Central Register and Information on Activities Economic, if a separate provisiony) require entry in the register or records.

6) Contractor's statement that he has not issued a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance contributions.

7) Contractor's statement about the lack of a decision against him as a preventive measure, the prohibition of applying for public ordersadvising the Contractor not to pay taxes and local fees, referred to in the Act of 12.1.1991 on taxes and local fees (Journal of Laws of 2016, item 716).

9) within 3 days from the date of posting on the website information referred to in art. 86 par. 5 of the Public Procurement Law, submits to the Ordering Party a declaration of belonging or non-membership in the same groupsy capital.

13. The offer binding period is 60 days.

14. The offer is a completed and signed Form "Offer" prepared on the basis of the template constituting Annex No. 1 to Volume I SIWZ IDW together with:

1) Price form prepared on the basis of the template constituting Annex No. 2 to Volume I SIWZ-IDW.

2) Technical specification offered subject of the lease, according to the model constituting the Annexk A to the Bid Form constituting Annex No. 1 to Volume I SIWZ IDW.

The Contractor is obliged to attach to the offer the valid statement on the day of the receipt of the offers and other statements or documents referred to in the ToR.
