1. The object of the order is Service and maintenance of the Paid Parking Zone in Ł-dź in 2019-2022.

1. The subject matter of the order is the service and maintenance of the Paid Parking Zone in Ł-dź in 2019-2022.

The subject of the order consists in the implementation of two tasks.

1.1.Zadanie 1-Equipment of Paid Parking Zone in Ł-dź (hereinafter referred to as SPP) in parking devices / parkomaty /, missing vertical markings defining the SPP area and places for paid parking and computers in itntrum of data processing with their servicing and maintenance.

1.2. Task 2 summer and winter, manual and mechanical cleansing SPP.

a) Types of services provided as part of summer maintenance:

1.Deleting any pollution "

3. Sweeping parking spaces

4. Washing out parking spaces

b) Types of services provided as part of winter maintenance:

1. Prevention and eradication-slipperiness

2. Snow removal of parking spaces

3. Winter cyclic purification

4. All activities related to summer cleaning - Detailed description of the order is included in Volume II of the SIWZ "Description of the subject of the Order for Task 1 and Task 2, Technical Specification for Task 2" and Volume III SIWZ

2. Employer pursuant to Article 29 para. 3a of the Act requires that the actand in the scope of order fulfillment, consisting of: - a) checking fees paid by drivers for a vehicle standstill in SPP,

reporting missing items on the SPP markings to the ordering party. They were performed by at least 10 persons employed under a contract of employment, if the performance of these activities consists in performing work in the manner specified in art. 22 -1 of the Act of 26.6.1974 - Labor Code (Dz.U.-from 2018, 917, as amended.)

B) - supervision over the technical condition of parking machines and mobile terminals,

- maintenance of devices,

- ongoing repair of defects,

- replenishment of paper for printing tickets,

- interventions in the case of failures reported by users were performed by at least 3 persons employed on the basis of a contract of employment, if the performance of these activities consists in the performance of work-in the manner specified in art. 22 § 1 of the Act of 26.6.1974 - Labor Code (Journal of Laws of 2018, item 917, as amended).

C) - providing drivers with information on the functioning of the SPP (by phone and in direct conversations) ) - accepting complaints from drivers and handing over letters to the contracting party, - printing reports on checks carried out, preparing them for shipment, enveloping and transmissionto the postal operator,

- supervision over the timeliness and course of cash-out from parking machines,

- supervision over the work of controllers and servants

They were performed by at least 2 persons employed under a contract of employment, if the performance of these activities consists in performing work in the manner specified in art 22 - 1 of the Act of 26.6.1974 - Labor Code (Journal of Laws of 2018, item 917 with.).

3. Requirements referred to in art. 29 para. 3a of the Act, concerning employment by the Contractor or subcontractor based on a contract of employment, persons performing the activities indicated by the Contracting Party in the scope of the order are set out in point 3.2. Volume I SIWZ and Volume III SIWZ.

The above requirements specify in particular:

a) the way of documenting the employment of persons referred to-in art 29a of the Act

b) the Awarding Entity's powers to control the fulfillment by the contractor of the requirements referred to in art. 29 para. 3a of the Act and sanctions for failing to meet these requirements, - c) the type of activities necessary to perform the order, which relate to the employment requirement based on a contract of employment by the contractor or subcontractor of a person performing activities in progress

The Employer stipulates that wherever in the Description of the Subject of the Order and the Detailed Technical Specification and attachments to them constituting the description of the subject of the order, they are indicated in the description of the subject (...) 1. This order based on art. 16 sec. 4 of the Public Procurement Law is granted jointly by: City of Łodź Zarząd Drog i i Transportu, ul. Piotrkowska 173, 90-447 Łodź and the Department of Municipal Economy in the Department of Social Affairs of the City of Ł-dź, 90-447 Łodź, ul. Piotrkowska 175.

1.1. Nand pursuant to the Regulation No. 8720 / VII / 18 of the Mayor of Ł-dź dated 22.6.2018 regarding the appointment of the Road and Transport Board to conduct the proceedings and grant a public order to ensure the operation of the paid parking zone in Ł-dź along with its maintenance designated to conduct the proceedings and awarding a public contract related to the implementationtasks covering the scope of tasks of the Road and Transport Board and the Department of Municipal Economy in the Department of Social Affairs of the City of Lodz Office is the City of Lodz - Road and Transport Management, 90- 447 Łodź, ul. Piotrkowska 173

2. Contractors may apply for the award of the contract, who will show no grounds for exclusion from the proceedings under Art. 24 sec. 1 of the Act (obligatory foundations ofkeygen) and art. 24 clause 5 of the Act (optional grounds for exclusion), meet, at the level described below, the conditions for participation in the proceedings referred to in art. 22 ust. 1 of the Act

Due to the limited number of characters in Section III.1.3), the award criteria, eligibility criteria, statements and documents to be provided by the Contractor, confirming compliance with the conditionsof participation in the proceedings and demonstration of the lack of grounds for exclusion have been specified in the documents of the order - Volume I SIWZ- Instructions for Contractors, posted on the website of the ordering party. Detailed information on the conditions of participation and grounds for exclusion and a list of statements or documents confirming the fulfillment of the conditions for participation in the proceedings and the lack of grounds for exclusion fromthey are present in points 6 and 7 of the ToR.

4. The contractor, at the latest when submitting the offer, sends in electronic form to the address of the Ordering Party (zamowienia@zdit.uml.lodz.pl) valid as of the day of submitting bids a statement constituting preliminary confirmation of compliance with the conditions of participation in the proceedings and no grounds for excluding the contractor from proceedings in the form a single order document (the so-called JEDZ). Contractor who refers to the resources of other entities, in order to demonstrate the lack of grounds for exclusion against them and fulfillment of the conditions of participation, to the extent in which he relies on their resources: - a. a single document, and

b. commitment of this entity to give back its stock for the needs of the contractor submitting the offer.

5. The Contractor, within 3 days of placing information on the opening of offers on the website, provides the Ordering Party with a declaration of belonging or non-membership in the same groupscapital referred to in art. 24 sec. 1 point 23 of the Act

6. The Contracting Authority provides for the possibility of using the order of art. 24aa paragraph 1 of the Act saying that: "The contracting authority may, in proceedings conducted under an open tender, first evaluate the offers, and then examine whether the contractor whose tender was rated as the most advantageous is not subject to exclusion andmeets the conditions for participation in the procedure "in accordance with the reservation in point 7 of I and SIWZ.

7 Upon ordering the Purchaser, in order to confirm the absence of grounds for exclusion of the Contractor from participation in the proceedings, the Contractor is required to submit information, certificates, a copy and statements of which referred to in -5 of the Regulation of the Minister of Development of 26.7.2016 on the types of documents that may be usede demand the Employer from the contractor in the proceedings for the award of the contract (Journal of Laws of 2016, item 1126)
