1. Contractors who are not subject to exclusion pursuant to art. May apply for awarding the contract. 24 paragraph 1 and art. 24 paragraph 5 point 1 itemawy Pzp.

2. Along with the offer, each Contractor is required to submit a JEDZ electronic statement with a qualified electronic signature:

- in the case of Contractors jointly applying for the contract, JEDZ shall submit each of the Contractors jointly applying for the contract,

- in the case where the Contractor in to confirm compliance with the conditions for participation in the proceedingsday relied on the abilities of other entities for each of these entities submits a separate EAT form and a written undertaking of these entities to make available to him the necessary resources for the implementation of the contract.

3. Each Contractor within 3 days of posting on the website the information referred to in art. 86 section 5 of the PPL Act submits declarations to the Employerie about belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act

4. In these proceedings, the Employer may apply the procedure provided for in Article 24 aa of the Public Procurement Law

5. The contractor whose offer will be rated the highest in the scope of a given task will be invited to submit to the designated entity before awarding the contract, not less than 10 days, within the following documents:

A. confirming fulfillment of the conditions for participation in the procedure, listed in part III, point 1.1 of the notice;

B. confirming the lack of grounds for exclusion from the procedure, i.e.: information from the National Criminal Record in the scope specified in art. 24 paragraph 1 item 13, 14 and 21 of the Public Procurement Law,

2) an excerpt from the relevant register or central registerentities and information on economic activity, if separate provisions require an entry in the register or records to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law,

3) the Contractor`s statement that he has not issued a final court judgment or final administrative decision regarding arrears with the payment of taxes, fees or insurance premiumssocial or health or in the event of the issuance of such a judgment or decision documents confirming the payment of these debts together with any interest or fines or the conclusion of a binding agreement on the repayment of these debts,

4) the Contractor`s statement that there was no decision against him as a preventive measure prohibiting him from applying for public procurement.

Indicated inpoint 1) 4) the documents are submitted by:

- the Contractor,

- in the case of Contractors jointly applying for the award of each of the Contractors jointly applying for the award of the contract,

- in the case where the Contractor, in order to confirm compliance with the condition of participation in the procedure will rely on the abilities of other entities indicated in item 1) 5) the documents the Contractor undertakeswill be submitted for each of these entities.

In order to confirm that the offered deliveries will meet the requirements specified by the Employer, it is required to:

- submit catalog sheets of the offered oil for task No. 2, if it is an equivalent product.

The offer must contain: completed offer form, completed price form, powers of attorney (if applicable).

The Contractor submits the offer forvia the purchasing platform.

Each Contractor entering the tender is obliged to submit a deposit before the deadline for submission of tenders, i.e. until 15/05/2020 by 09.30.00, in the amount specified for the task.
