Object of the procedure is the purchase of oxygen for 24 months. Medical oxygen cylinders rental with built-in pressure regulator, medical oxygen cylinders rental without pressure regulator, ISCIR cylinder check and authorization, valve replacement, painting of oxygen cylinders

I) Exclusion reasons:

1) The bidders, the supporting third parties and the subcontractors must not be present in the situations stipulated in art. 60, 164, 165, 167 of Law no. 98 / 2016.

The manner by which the fulfillment of the requirement can be demonstrated:

- DUAE will be completed by the eco operatorsnominees participating in the award procedure with information related to their situation. The supporting documents proving the fulfillment of the assumed by completing the DUAE are to be submitted, at the request of the contracting authority, only by the tenderer placed on the first place in the intermediate rankings drawn up at the completion of the evaluation of the tenders.

These documents must contain informationand may be:

(a) certificates showing the absence of debts with respect to the payment of taxes, duties or contributions to the general consolidated budget (local budget, state budget, etc.) outstanding at the time of presentation; cf. art. 196 of Law 98/2016,

(b) the criminal record of the economic operator and the members of the management, management or supervisory body of the(c) as the case may be, documents proving that the economic operator (s) has the power to represent, decide or control over it, as evidenced by the certifying certificate issued by the ONRC / may benefit from the derogations provided for in art. 166 par. (2), art. 167 par. (2), art. 171 of Purchasing Law 98/2016(d) other editing documents, as the case may be.

The decision-makers of the contracting authority involved in the public procurement procedure are: - - Dr. Venter Liciniu Catalin (general manager), - dr. 1)The certificate issued by the Trade Register Office at the place of the competent court or by the presentation of the certifying certificate issued by the ONRC showing that the object of the contract has a correspondent in the authorized CAEN code, according to Law 359/2004.

The economic operators submitting the tender must prove a form of registration under the law of the country of residenceto prove that the economic operator is legally constituted, that he is not in any of the situations of cancellation of establishment and that he has the professional capacity to carry out the activities which are the object of the contract. The way in which the fulfillment of the requirement can be demonstrated: it will be completed by the economic operators participating in the award procedure with informationtheir situation. The supporting documents proving the fulfillment of the assumed by completing the DUAE, respectively the certificate issued by the ONRC or, in the case of foreign tenderers, equivalent documents issued in the country of residence shall be submitted, at the request of the contracting authority, only by the tenderer ranked I in the intermediate rankingswhen bid evaluation is completed.
