Upper and lower stage mechanization modernization service

The subject of the contract is the lower and upper stage mechanization modernization service, divided into 2 parts:

Main Site: Opera Krakowska, ul. Lubicz 48, 31-512 Krakow, POLAND.

The subject of the contract and the conditions for its implementation for Part I in the field of modernization of the mechanization of the lower stage were specified in Szczegoł-wm the description of the subject of the order, constituting Annex 1 to the ToR and in the model contract constituting Annex 7a to the ToR, based on the specifications and studies listed below: 200 cm, which will allow artists to enter under the trap "; constituting Annex No. 1A to the ToR;

-" DesignIany stage floor located on the main stage, proscenium and in the left stage pocket together with the correction of tracks and the system of running side stage carts "; constituting Annex No. 1B to the Terms of Reference.

Contractors who meet the conditions of participation in the procedure regarding economic or financial standing, i.e. they show that they are insured against repaymentcivil liability in the scope of business activity related to the subject of the contract, for an insurance sum of at least PLN 200, 000.00 both for part I of the contract and part II of the contract. . 24 paragraph 1, and will exclude the Contractor from the procedure pursuant to art. 24 paragraph5 points 1 and 8.

Public Procurement Law (Journal of Laws of 2017, item 1579 and 2018), hereinafter referred to as "PPL Act";

your personal data will be stored in accordance with Article 97 para. 1 of the Public Procurement Law, for a period of 4 years from the date of termination of the contract award procedure, and if the duration of the contract exceeds 4 years, the storage period covers the entire timetermination of the contract;

The obligation for you to provide personal data directly regarding you is a statutory requirement specified in the provisions of the Public Procurement Law related to participation in a public procurement procedure; the consequences of not providing specific data result from the Public Procurement Law;

With regard to your personal data, decisions will not be taken in an automated manner, applying to art. 22 GDPR;

You have: - based on art. 15 GDPR, the right to access your personal data regarding you;

- pursuant to art. 16 GDPR, the right to rectify your personal data;

- pursuant to art. 18 GDPR, the right to request the administrator to limit the processing of personal data, subject to the cases referred to in art. 18 clause 2 GDPR;
