The subject of the order in these proceedings is the successive delivery of endoproalso and implants and other orthopedic materials for the SP ZOZ Multospecialist Hospital in Jaworzno.

The subject of the order is the gradual delivery of adjacent endoprostheses.

The orderer does not impose any requirements in this respect.

The Employer will exclude the Contractor from the public procurement proceduretowards which there are prerequisites for exclusion, referred to in art. 24 paragraph 1 Public Procurement Law In addition, from Art. 24 paragraph 5 item 1 of the Public Procurement Law, the Awarding Entity will exclude the Contractor from the public procurement procedure: in relation to which liquidation has been opened, the arrangement approved by the court in the restructuring proceedings provides for satisfaction of creditors by liquidation of its assets or ared ordered liquidation of his assets pursuant to art. 332 paragraph 1 of the Act of 15.5.2015 Restructuring Law (Journal of Laws of 2015, item 978, 1259, 1513, 1830 and 1844 and of 2016, item 615) or whose bankruptcy was announced, with the exception of the Contractor who after declaration of bankruptcy has concluded an arrangement approved by a final court decision, if it systematically provides for satisfaction of creditors by liquidation of propertytowards the bankrupt, unless the court ordered the liquidation of his property pursuant to art. 366 section 1 of the Act of 28.2.2003 Bankruptcy Law (Journal of Laws of 2015, item 233, 978, 1166, 1259 and 1844 and of 2016, item 615).

The Contractor is obliged to submit along with the offer:

1. completed offer form (Annex 1 to the ToR) and product and price listings (respectively on Annexes 3.1 to 3.20);

2. DECLARATIONnot that the offered subject of the order is consistent with the description contained in Annex 3 to the ToR;

3. list of scientific publications (including the source) confirming the solution / parameters offered as part of the adopted assessment criterion, if the Contractor has declared their possession (in the absence of a list, the Contractor will receive 0 points).

4. Proof of payment of the bid security

Bidder obliged to offery is to attach: a current statement as at the date of submission of offers constituting a preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions for participation in the procedure in the form of a single European order document, drawn up in accordance with the template of the standard form set out in the European Commission implementing regulation issued on the basis of Article 59 paragraph. 2 of the Directive014/24 / EU, hereinafter referred to as the "Single Document". The Contractor is obliged to submit a single document to the extent indicated in Annex 2 to the ToR. 10 days, valid on the date of submitting statements or documents confirming the circumstances, referred to in art. 25 paragraph 1 of the Public Procurement Law. (described in detail in chapter V, section 8 of the ToR). A detailed list of documents is in the ToR. The contracting authority foresees in the model contracts the Contract changes as well as the terms and circumstances of the contract change.
