Main Site: Regional Health Service Companies (SSR) of Friuli-Venezia Giulia (FVG).

Substitutive declaration of certification, pursuant to art. 80 of Legislative Decree 50/2016, with which the company certifies that it is not illegalin any of the exclusion conditions provided for in the article itself, or in other situations which, in accordance with current legislation, constitute grounds for impediment to participation in the tender and / or for the conclusion of the contract in the event of an award as well as being registered in the register of the Chamber of Commerce, Industry, Craft and Agriculture (CCIAA), where foreseen (or in a registrict of the firms or analogous professional registers for the foreign States, etc.), specifying the place and the number of registration to the register of the enterprises and that to load of the firm there are not declaration of bankruptcy, forced liquidation, concordato preventive, ne a proceeding for the declaration of one of these situations is underway at its expense.
