The following documents and declarations should be attached to the offers:

1. Completed and signed by authorized persons to represent the contractorthe offer form, prepared according to the model constituting Annex 1 to the specification.

2. Completed and signed by the persons authorized to represent the contract Price form, prepared according to the template constituting Annexe 1A to the specifications prepared on the basis of the description of the subject of the order (Annex 1B). (The form should contain all possible changes made in timeduration of proceedings)

3. Proof of payment / payment of a deposit / original guarantee

4. In order to demonstrate the elements of the Contractor's compliance with the conditions of participation in the procedure and the lack of grounds for exclusion, the Contractor shall submit in the form of a Single Form European Document prepared in accordance with the standard form set out in the Executive Order of the Commissionand the European one issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as "single document" or "EAT" acc. a design, submitted only using means of electronic communication bearing a qualified electronic signature - attached as Annex 2.

5. In the event of a joint application by the Contractors (consortium), a single document (EAT) shall be submitted to eachy from Contractors jointly applying for the order. These documents confirm the fulfillment of the conditions for participation in the proceedings, no grounds for exclusion, in which each of the Contractors demonstrates fulfillment of the conditions for participation in the proceedings, no grounds for exclusion. In third party entities, the EAT of the third party is submitted by the Contractor if it refers to the resources of third parties in order to demonstrate-compliance with the conditions for participation in the proceedings. EAT should be completed to the extent that the Contractor uses the resources of the third party. EAT should also refer to the verification of the grounds for exclusion

7. Power of attorney if the offer is not signed by an authorized person and shown in the National Court Register.

8. According to art. 24aa paragraph 1 Pzp, the Purchaser will first evaluate the offers and then examine whether youthe contractor whose offer has been assessed as the most advantageous, is not subject to exclusion and meets the conditions for participation in the proceedings. The list of statements and documents is specified in Chapter 7 of the ToR.
