The subject of public procurement is the PIPAC pump supplier for I. CHK General University Hospital in Prague, including accessories, duties, transport, installation, commissioning, initial validation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (further only Act No. 268/ 2014). training of the purchaser and handing over of the documents relating to the goods to be supplied, especially the declaration of conformity and instructions of the shop in the printed and electronic form, including the guarantee service and the other conditions of the documentation. The subject of public procurement is the supply of PIPAC pumps for the 1st CHK General Faculty of Hospitalsincluding in Prague, including duties, customs, transport, installation, putting into operation, entrance validation, execution of instructions according to the relevant provision of Act No. 268/2014 Coll. on Medical Devices (Act No. 268/2014 Coll. training of the buyer and handing over of the documents relating to the goods to be supplied, in particular the declaration of conformity and the instruction of the caravan in the language printed by himincluding a warranty service, and in accordance with other conditions of the filing documentation.

(a) Direct link in which it is possible to verify the entry in the Commercial Register at https://www.justice.cz/, if it is registered or listed, or shall make a reference to another similar record in which the supplier is registered under a special legal regulation (Section 77 (1) of the Act). The Supplier shall attach a declaration of honor, that registered information is up to date and true. The contractor can prove the capability and also the document itself in electronic form;

b) A direct link in which it is possible to verify the authorization to carry out business according to special legal regulations in the scope corresponding to the subject of the public offer on http://www.rzp.cz/, a document proving the right of a trade license or license (-Article 77 (2) a) the law). The Supplier shall affirm honestly that the registered information is up to date and true. The contractor can prove the capability and also the document itself in electronic form;

c) A direct link in which it is possible to verify the registration at http://www.sukl.cz/, which authorizes it to distribute and service the medical devices (if provided device healthytical means according to Act No. 268/2014 Coll.). This registration must be valid as of the date of submission of the offer and the selected supplier must keep it in force for the entire period of the subject of the public offer. The Supplier shall affirm honestly that the registered information is up to date and true. The supplier can prove this capability also by documenting the document itself in electronic form
