ZP / 32/2020 Delivery and assembly of furniture for organizational units of the Medical University of Gdańsk

1. The subject of the order is the delivery and assembly of furniture for the organizational units of the Medical University of Gdańsk located in the Tri-City.

Main Site: The organizational units of the Medical University of Gdańsk arrangethose in the Tri-City, POLAND.

Contractors who can: apply are not eligible for exclusion; meet the conditions for participation in the procedure, the contracting authority does not set specific conditions in this respect. 24 paragraph 1 point 12 23. Contractor`s exclusion will followpi: in the cases referred to in art. 24 paragraph 1 point 13 lit. a c and point 14, when a person referred to in these provisions has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. a c, if 5 years have not elapsed since the day the verdict confirming the existence of one of the grounds for exclusion became final, unless another exclusion period was specified in that judgment; in cases of whoreferred to: in art. 24 paragraph 1 point 13 lit. d and point 14, when a person referred to in these provisions has been convicted of an offense listed in art. 24 paragraph 1 point 13 lit. d, in art. 24 paragraph 1 point 15, in the cases referred to in art. 24 paragraph 1 points 18 and 20, in the case referred to in art. 24 paragraph 1 point 21, and in art. 24 paragraph 1 point 22. The contracting authority also provides for the exclusion of an economic operator on the basis ofknows art. 24 paragraph 5 point 1. The awarding entity, prior to awarding the contract, as regards demonstrating the lack of grounds for exclusion, will call the contractor whose tender has been rated the highest to be submitted within a designated period, not shorter than 10 days, valid as at the date of submitting the following statements or documents: information from the National Register Criminal within the scope specified in art. 24 paragraph 1 point 13, 14 and21 of the Act, issued not earlier than 6 months before the deadline for submitting bids; an excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate provisions require an entry in the register or a register, to confirm the absence of grounds for exclusion pursuant to art. 24 paragraph 5 point 1 of the Public Procurement Law; Contractor`s declaration of no release against himan final court judgment or final administrative decision on arrears in payment of taxes, fees or social security or health insurance contributions or in the case of issuing such a judgment or decision documents confirming the payment of these claims together with any interest or fines or concluding a binding agreement on the repayment of these claims ; say toContractor`s decision on the lack of a decision against him as a precautionary measure forbidding public procurement (requirement described in more detail in the ToR). 5. If the contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to:

1. in paragraph 4 point 1 above, submits information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial or administrative authority of the country in which the contractor has its registered office or place of residenceof residence or place of residence is the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 item 13, 14 and 21 of the PPL Act. This document should be issued not earlier than 6 months before the submission deadline;

2. in paragraph 4 point 2 above, submits the document or documents issued in the country in which the contractor has its registered office or place of residence, confirmaccordingly that its liquidation has not been opened or its bankruptcy has been declared. This document should be issued not earlier than 6 months before the deadline for submitting bids.

If in the country where the contractor has its registered office or place of residence or place of residence there is a person to whom the document relates, no documents referred to in paragraph 5, are replaced by the document containsthe contractor`s statement, indicating the person or persons authorized to represent him, or the statement of the person to whom the document was intended, submitted before a notary public or before a judicial, administrative or professional or economic self-government body competent for the seat or place of residence of the contractor or place of residence this person. Date of issueo the document should comply with the relevant dates referred to above;

6. the contractor within 3 days from the date of posting on the purchasing platform the information referred to in art. 86 section 5 of the Public Procurement Law, will provide the contracting authority with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the PPL Act. With the submission of the declaration, usethe principal may provide evidence that the links with another contractor do not lead to a distortion of competition in the procurement procedure;

7. the contractor is not obliged to submit declarations or documents confirming the circumstances referred to in art. 25 paragraph 1 item 1 and 3 of the Act, if the contracting authority has declarations or documents regarding this contractor or can obtain them usingfree and publicly available databases, in particular public registers within the meaning of the Act of 17 February 2005 on the computerization of the activities of entities performing public tasks (Journal of Laws of 2014, item 1114 and of 2016, item 352).
