The subject of the orders is the delivery of furniture to the Department of Neurology and Impact Department with Early Neurological Rehabilitation in the Voivodship Multispecialty Center of Oncology and Traumatology them. M. Kopernika in Łodzi (62 Pabianicka St.), in accordance with the requirements specified in Annex 1.1 and 1.2 to the ToR. The ordering party does not allow partial offers. The offer must cover the entire subject of the order.

The subject of the order is the delivery of furniture equipment included in the integral part of the description of the order which constitutes Annex 1.1 and 1.2 to the ToR. - From the proceedings for grantingNo ordering The contracting authority may exclude a contractor in relation to which the liquidation has been opened, the court approved in the restructuring proceedings is expected to satisfy the creditors by liquidating its assets or the court ordered the liquidation of its assets pursuant to art. 332 par. 1 of the Act of 15.5.2015. Restructuring law (Journal of Laws of 2016, item 1574, 1579, 1948 and 2260) or whose bankruptcy was announced, with the exception of the contractor who, after the bankruptcy declaration, concluded an agreement approved by a final court decision, if the arrangement does not provide for satisfying the creditors by liquidating the assets of the bankrupt, unless the court ordered the liquidation of its property pursuant to art. 366 para. 1 of the Act of 28 February 2003. Bankruptcy Law (Journal of Laws of 2016, item 2171, 2260 and 2261 andz from 2017 item 791) (Article 24 paragraph 5 point 1 of the Public Procurement Law).

The Contractor is obliged to submit: The Contractor`s declaration as per the day of submitting offers in the mode of art. 25a para. 1 of the PPA in the form of a standard form of the Single European Order Document, constituting Annex No. 3 to the ToR. - Documents confirming no grounds for exclusion:

1. Information from the National Criminal Record inthe scope specified in art. 24 sec. 1 point 13 of the Public Procurement Law,

2. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 14 of the PPL,

3. The Contractor`s statement that he has not been the subject of a final court judgment or final administrative decision against his payment of taxes, fees or social security or health insurance premiums or in the event ofof the judgment or decision, documents confirming the payment of these claims together with any interest or fines or the conclusion of a binding agreement regarding the repayment of such claims. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 21 of the Public Procurement Law

5. Declaration of the contractor about the lack of a decision against him as a precautionary measure for the ban on whiplashapplying for public orders.

1. In the case of joint application for contractors, a uniform document in the form of a standard form of the Single European Order Document constituting Annex No. 3 to the ToR shall be submitted by each of the contractors jointly applying for the order. These documents confirm the lack of grounds for exclusion to the extent that each of the contractors inshows the lack of grounds for exclusion, in accordance with art. 25a para. 6 Public Procurement Laws

2. According to art. 23 par. 3 of the Public Procurement Law, provisions of the SIWZ regarding the Contractor shall apply accordingly to contractors jointly applying for the award of the order. Contractor`s exclusion follows:

a) in the cases referred to in art. 24 sec. 1 point 13 lit. a c and point 14 of the Public Procurement Law, when the person referred to in these provisionsPermanently convicted of the offense listed in art. 24 sec. 1 point 13 lit. a c) of the PPL, if it has not been 5 years since the judgment confirming the existence of one of the grounds for exclusion has become final, unless a different period of exclusion has been specified in that judgment; 24 sec. 1 point 13 lit. d and point 14 of the PPL Act, when the person referred to in these provisions, fromhas been sentenced for the offense listed in art. 24 sec. 1 point 13 lit. d of the Public Procurement Law,

in art. 24 sec. 1 point 15 of PPL Act;

c) if it has not been 3 years since the judgment confirming one of the grounds for exclusion has become final, unless a different period of exclusion has been specified in this judgment or from the date on which the decision confirming one of the grounds excludes(a) in cases referred to in Article 24 sec. 1 items 18 and 20 of the Public Procurement Law, if 3 years have elapsed since the occurrence being the base for the exclusion, e) in the case referred to in art. 24 sec. 1 point 21 of the Public Procurement Law, if the period for which the prohibition of applying for public orders has not been validly terminated, f) in the case referred to in art. 24paragraph. 1 point 22 of the Public Procurement Law, if the ban on applying for public orders has not expired.

3. Contractor who is subject to exclusion pursuant to art. 24 sec. 1 point 13 and 14 and 16 20 or art. 24 sec. 5 of the Public Procurement Law, may provide evidence that the measures taken by it are sufficient to demonstrate its reliability, in particular to prove that the damage caused tofiscal offense or crime, financial compensation for damage suffered or rectifying the damage, comprehensive explanation of the facts and cooperation with law enforcement agencies and taking specific technical, organizational and human resources that are appropriate to prevent further offenses or fiscal offenses or improper conduct of the contractor.The provision of the first sentence shall not apply if the contractor, being a collective entity, has been the subject of a final court decision prohibiting him from applying for the order and the period of validity of the prohibition specified in that judgment has not expired. The Contractor is not subject to exclusion if the awarding entity, considering the weight and special circumstances of the contractor`s action, deems sufficient evidencepresented on the basis of point V, b. 3 SIWZ.

2. In cases referred to in art. 24 sec. 1 point 19 of the Public Procurement Law, before the contractor is excluded, the contracting authority provides the contractor with the opportunity to prove that his participation in the preparation of the contract award procedure will not distort competition.

3. Contractor, within 3 days of posting information on the website referred to inArt. 86 point 5 of the Public Procurement Law, it provides the orderer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law Act, together with the submission of a statement, the contractor may provide evidence that links with another contractor do not lead to distortion of competition in the contract award procedure (Annex No. 5 to the Terms of Reference).shall request the deposit of the bid bond by the Contractors participating in the proceedings. The bid security for the entire subject of the order is PLN 11, 500.00 in words: eleven thousand five hundred zlotys 00/100. The total amount of the bid bond is the sum of the tender bond amounts:
