Purchase routers for WAN LP

Reference Number: DZ.270.59.2019

1. The subject of the order is the delivery and implementation of 4 pieces of network routers in the Primary Center and the State Forests backup center. The scope of services includes delivery, installation, commissioning and configuration of systems as well as workshops for the administrators of the Lasow Pańs networkin accordance with the Description of the subject of the order constituting Annex No. 1 to SIWZ.

1. The subject of the order is the delivery and implementation of 4 pieces of network routers in the Primary Center and the State Forests backup center. The scope of services includes the delivery, installation, commissioning and configuration of the systems and the implementation inarsztatow for the administrators of the State Forests network, in accordance with the Description of the subject of the order constituting Annex No. 1 to the ToR.

2. The contractor is obliged to execute the order under the terms and conditions described in the contract template constituting Annex No. 4 to the ToR.

Employer in accordance with art. 8a paragraph 1 of the Act of 29.1.2004 Public Procurement Law (public (ie Journal of Laws of 2018 item 1986 with later amendments) informs that when granting a public contract, the provisions of the Regulation of the European Parliament and of the Council (EU ) 2016/679 of 27.4.2016 on the protection of natural persons in connection with data processingpersonal data and regarding the free flow of such data and the repeal of Directive 95/46 / EC (general regulation on data protection) to the extent specified in the Terms of Reference.

The Contractor is obliged to submit a bid bond before the deadline for submission of tenders in the amount of PLN 45, 000.00 (in words: forty-five thousand zlotys).

The ordering party must submit an offer together with the offerat the initial confirmation of fulfillment of the conditions for participation in the procedure and no grounds for exclusion of the Single European Order Document (EJM).

The contracting party shall call the Contractor whose tender has been the highest rated, to submit in the shortened day, not shorter than 10 days, the current date submit the following declarations or documents:

3. Information from the National Criminal Register in the scope specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids in the proceedings,

4. certificate of the competent head of the tax office confirming that the Contractor is not in default of paying taxes, issued not earlier than 3 months before the expiry ofm the deadline for submission of bids or other document confirming that the Contractor has entered into an agreement with the competent tax authority regarding repayment of such receivables with possible interest or fines, in particular obtained legally permissible, postponement or payment of overdue installments or suspension in full of the implementation of the decision authority;

5. certificate of ownershipthe off-site organizational unit of the Social Insurance Institution or the Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of offers or other document confirming that the Contractor has concluded an agreement withthe competent authority on the repayment of such claims, together with any interest or fines, in particular obtaining the legally required exemption, deferral or payment in installments of overdue payments, or withholding in full the execution of the decision of the competent authority; excerpt from the relevant register or from the Central Register and Information on Economic Activity, if separate regulations are requiredits entry in the register or records, in order to confirm the absence of grounds for exclusion under Art. 24 sec. 5 point 1 of the Public Procurement Law,

7. the Contractor's statement that he has not been the subject of a final court judgment or final administrative decision on default of taxes, fees or social security or health insurance premiums or in the event of such a decision or decisiondocuments confirming the payment of these claims together with any interest or fines or a binding agreement on repayment of such receivables; the Contractor's statement about the lack of a ruling against him as a preventive measure of the prohibition against applying for public orders; - 9. Contractor's knowledge of not paying taxes and local fees, of whichreferred to in the Act of 12.1.1991 on local taxes and charges (Journal of Laws of 2018, item 1445),

The Purchaser requires a performance bond to be provided in the amount of 10% of its maximum gross value.
