Maintenance work on watercourses in the administration of the Nysa Basin Administration

The subject of the order is to remove lichen from the escarpment with raking or fragmentation of swath, cutting down and grubbing up shrubs, cleaning the area of remains after rakingwith removal and utilization. Extraction from the watercourse of lichen plants rooted in the bottom (hacking), removal of blockages, cleaning outlets and culverts. Manual and mechanical desludging of the bottom of the watercourse, de-agglomeration of spoil after mechanical desludging.

The objective of the task is to restore patency and flow capacity of the riverbed on the Borkowicki River 0, 000 - 5, 200 in the town of Skorogoszcz - Niwy by removing the growing vegetation of thick and hard lichen. Removal of vegetation from the bottom by hand hacking, and removal of bushes along with mechanical chopping.

Contractors who can meet the requirements: who meet the conditions for participation in the procedure and are not subject to exclusion in the circumstances referred to in art. 24 paragraph 1 of the PPL Actin order to confirm the lack of grounds for excluding the Contractor from participation in the procedure, the contracting authority requires the following documents: 1 information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13 of the PPL Act issued not earlier than 6 months before the deadline for submitting bids in the case of a Contractor being a natural person;

2. information from the National Criminal Register regardingspecified in art. 24 paragraph 1 point 14 of the Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids in relation to: a) an active member of the Contractor`s management body, b) an active member of the Contractor`s supervisory body, c) a partner in a public company in a public or partner company, d) general partner in a limited partnership or limited joint-stock partnership, e) a proxy;

3information from the National Criminal Register in the scope specified in art. 24 paragraph 1 point 21 of the Public Procurement Law issued not earlier than 6 months before the deadline for submitting bids in the case of the Contractor who is a collective entity;

4 statements by the contractor that he has not issued a final court judgment or the final administrative decision regarding arrears with the payment of taxes, fees or contributionsfor social or health insurance or in the event of the issuance of such a judgment or decision of documents confirming the payment of these receivables together with any interest or fines or the conclusion of a binding agreement on the repayment of these receivables;

5 the contractor`s statement that he has not been ordered to prevent him from applying on public procurementical;

The Employer requests from the Contractor, which relies on the abilities or situation of other entities on the principles set out in Art. 22a of the Public Procurement Law, submitting documents referred to in point 1-5

6 If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in points 1-3 submits information with the appropriateego register or, in the absence of such a register, another equivalent document issued by a competent judicial or administrative authority in the country in which the contractor has its registered office or place of residence or place of residence, has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21;

If in the country in which the contractor has its registered office or place of residencethe person to whom the document relates is domiciled or the place of residence does not appear, the documents referred to in point 6 shall be replaced by a document containing a statement of the contractor, indicating the person or persons authorized to represent him, or a statement of the person to whom the document was to relate, submitted before a notary public or before a judicial, administrative or self-governing bodyprofessional or economic government competent for the registered office or place of residence of the contractor.

6.3.9 The contractor having its registered office in the territory of the Republic of Poland, in relation to a person residing outside the territory of the Republic of Poland, to whom the document referred to in point 2-3 submits the document referred to in item 6., to the extent specified in art. 24 paragraph 1 point14 and 21. If, in the country in which the person to whom the document was to reside, does not appear such documents, it is replaced by a document containing the person`s declaration made before a notary public or before a judicial, administrative or professional or economic self-government body because of the person`s place of residence ..

Documents referred toin points 6-7 of this notice in section III.1.1. should be issued not earlier than 6 months before the submission of tenders.

Contractors within 3 days of posting on the website referred to in art. 86 section 5 of the Public Procurement Law, provides the Employer with a declaration of belonging or not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 Pzp. With the submission of the statement, the Contractor may provide evidence that links with another Contractor do not lead to a distortion of competition in the procurement procedure.

Pursuant to art. 24aa of the Public Procurement Law, the Awarding Entity informs that in these proceedings it will first evaluate the offers and then check whether the Contractor whose offer has been assessed as the most advantageous subject toexclusion and meets the conditions for participation in the proceedings.

Pursuant to art. 25 a paragraph Public Procurement Law the contractor together with the offer submits a current statement as at the date of submission of the offer to the extent indicated by the contracting authority in the contract notice or the specification of the essential terms of the contract in the form of a single European order document prepared in accordance with the template of the standard form specifiedin the European Commission implementing regulation issued on the basis of Article 59 (2) of Directive 2014/24 / EU, hereinafter referred to as "EAT", the information contained in JEDZ constitutes preliminary confirmation that the contractor is not subject to exclusion and meets the conditions for participation in the proceedings. electronic form with a qualified electronic signature, in accordance with the provisions specified in the ToRZ
