2. Documents submitted to the ordering party's request:

a) excerpt from the relevant register or central records and business information, if separate regulations require an entry in the register or records, in order to demonstrateto grounds for exclusion based on art. 24 sec. 5 point 1 of the Public Procurement Law, b) information from the National Criminal Register, to the extent specified in art. 24 sec. 1 point 13, 14 and 21 of the Public Procurement Law, issued no earlier than 6 months before the deadline for submitting bids;

c) the contractor's statement that the contractor has not been the subject of a final court judgment or final administrative decision aboutpayment of taxes, fees or social security or health insurance premiums or - in the event of such a verdict or decision - documents confirming payment of such claims together with any interest or fines or conclusion of a binding agreement on repayment of such claims; d) a statement by the contractor about the lack of a ruling against him as a measure of a zapobforbidding to apply for public orders.

3. Foreign contractors:

If the contractor is established or resides outside the territory of the Republic of Poland, instead of the documents referred to in point 2 lit. a) -b) submit, respectively, the documents indicated in point 7.5.1. SIWZ.

The Ordering Party does not describe, does not set a specific condition in this respect.

1.1. The offer must be securedon the bid bond in the amount of: - - partial task No. 1 PLN 50.00 (in words: fifty zlotys 00/100),

- partial task No. 2 200.00 PLN (in words: two hundred zlotys 00/100).

1.2. The bid bond should be submitted before the deadline for submitting offers.

1.3. The bid bond can be contributed in one or more of the following forms:

- in money:

- by bank transfer to the Employer's bank account:

2. The Contracting Party requests that the Contractor submits with the offer:

a) A form filled out and signed by a person authorized to submit declarations of will on behalf of the Contractoroffer - according to Annex No. 1 to the Terms of Reference;

b) Uniform European Order Document;

c) Technical description of the offered cameras along with an indication of all technical parameters, to assess the fulfillment of the Employer's requirements specified in the ToR;

d) Power of Attorney or other document, from which the right to sign the offer and other documents submitted together with the offer results;

e) Proof of payment of the bid security - Within 3 days from the date of placing by the Ordering Party on the website information on the opening of bids, referred to in art. 86 par. 5 of the Public Procurement Law, the Contractor is obliged to provide the Employer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. With the submission of statementsia, the Contractor may provide evidence that links with another contractor do not lead to distortion of competition in the procurement procedure.

In the case of joint application for contractors, a declaration of affiliation or non-affiliation to the same capital group is submitted by each contractor. 3) The Contractor submits the offer in electronic form via Platformy at https://e-ProPublico.pl/.

4) The offer and statement of EAT, are made, under pain of nullity, in electronic form and are provided with a qualified electronic signature.

5) The power of attorney must be submitted in electronic form, signed with a qualified electronic signature or electronic copy authenticated by a notary.

6) Documents or statementsreferred to in the Regulation on the types of documents that the contracting authority may request from the contractor in the procedure for awarding the contract (Journal of Laws of 2016, item 1126 as amended), are submitted in original in the form of an electronic document or in an electronic copy of the document or a statement certified for compliance with the original. Certification of compliance with the original electronic copy of the documentu or a statement is made using a qualified electronic signature.
