Provthe subject of the order in this proceeding is the delivery and implementation of the IT system providing online services along with the development of the electronic medical records system (EDM) described in Attachment 1 to SIWZ

Main Site: SPZOZ MSWiA z W-MCO w Olsztynie, Al. Wojska Polskiego 37, 10-228 Olsztyn, POLAND. The subject matter of the orders in this procedure is delivery and implementation-an IT system providing on-line services along with the development of the electronic medical records system (EDM) described in Annex No. 1.

The contractor will meet the condition if it proves that he is insured against civil liability for business activities related to the subject of the order for a guarantee sum of at least 2 000, 000.00 PLN

The ordering party requirespayment of the tender bond for the amount of PLN 35, 000.

II. The orderer anticipates the use of the so-called the "return" procedure in accordance with Article 24aa of the Public Procurement Law

III The Ordering Party does not provide for supplementary orders referred to in Article 67 paragraph 1 items 6 and 7 of the Pzpi Act, Article 134 para. 6 points 3.

IV. The Employer informs that he does not intend to conclude a framework agreement, he does not intend to establish a dynamic purchasing system, nothe will discuss the use of the electronic auction.

V. The Ordering Party will exclude from the proceedings of the Contractors based on the premises resulting from art. 24 sec. 1pt. 12 -23 and art. 24 sec. 5 points 1 and 8 of the Public Procurement Law - VI. In order to demonstrate the absence of grounds for exclusion, the Contracting Authority requests that the Contractors submit an EAT (initial declarations) and documents referred to in § 5 points 1-6 and 9 and 10 of the Minis Regulation.tra Development of 26.7.2016 on the types of documents that the contracting authority may request from the contractor in the proceedings for the award of the contract.

VII. In order to demonstrate fulfillment of the conditions for participation in the proceedings, the Contracting Party requests that the Contractors submit the JAG and the documents referred to in - 2 para. 4 points 2 and 9, 10 of the Regulation of the Minister of Development of 26.7.2016 on types of documentsmay be requested by the contracting party from the contractor in the procurement procedure
