The contractors can apply for contractors who are not subject to exclusion and arethey fulfill the conditions in the proceedings.

1. The contractor is excluded from the contract award procedure in respect of which any of the circumstances referred to in Article 24 sec. 1 point 12-23 of the Public Procurement Law - 2. In addition, the orderer will exclude the contractor in relation to which any of the circumstances referred to in Article 24 sec. 5 Public Procurement Laws

3. The ordering party, in accordance with art.-24aa of the Public Procurement Law, will first evaluate the offers, and then examine whether the Contractor whose tender has been evaluated as the most advantageous is not subject to exclusion and meets the conditions for participation in the proceedings. The Contractor is obliged to submit a statement valid as of the day of submitting bids constituting preliminary confirmation that the Contractor is not subject to exclusion and meets the conditions of participationu in the proceedings.

5. The statement referred to in point 4 The Contractor is obliged to submit in the form of a single document drawn up in accordance with the specimen of the standard form defined in the European Commission's implementing regulation issued on the basis of art. 59 par. 2 of Directive 2014/24 / EU, hereinafter referred to as "single document".

6. Contractor, within 3 days from the date of placing on the page ande of internet information referred to in art. 86 par. 5 of the Public Procurement Law, provides the orderer with a declaration of belonging or non-affiliation to the same capital group as referred to in art. 24 sec. 1 point 23 of the Public Procurement Law. With the submission of a statement, the Contractor may provide evidence that links with another Contractor do not lead to distortion of competition in the proceedings for grantingspeech.

7. At the request of the ordering party, the Contractor is obliged to submit the following declarations or documents:

10. In order to confirm the lack of grounds for excluding the Contractor from participation in the proceedings:

1) information from the National Criminal Register in the scope specified in art. 24 sec. 1 items 13, 14 and 21 of the Public Procurement Law and, regarding a conviction for a prison sentence, to the extent specified by the orderpursuant to Article 24 sec. 5 points 5 and 6 of the Public Procurement Law, issued not before

Six months before the deadline for submitting bids,

2) certificate of the competent head of the tax office confirming that the Contractor is not in arrears with taxes, issued not earlier than 3 months before the deadline for submission offers or other document confirming that the Contractor has entered into an arrangementwith the competent authority regarding the repayment of such receivables with possible interest or fines, - In particular, obtained legally permissible, postponement or payment of overdue payments or suspension in full of the decision of the competent authority; - 3) certificate of the relevant organizational unit of the Insurance Company Social or Agricultural Insurance Fundia Social or other document confirming that the Contractor is not in arrears with paying social security or health insurance contributions, issued no earlier than 3 months before the deadline for submission of tenders or other document confirming that the Contractor has concluded an agreement with the competent authority regarding repayment of these receivables together with possible interest or fines, in particular(a) an excerpt from the relevant register or from central records and business information, if separate regulations require an entry in the register or records, in order to provide for the exemption, deferral or installment of overdue payments in installments; demonstrating the lack of grounds for exclusion under Art. 24 sec. 5 point 1 usTawy Pzp.

5) Contractor's statement that he has not issued a valid court judgment against him or Or a final administrative decision on late payment of taxes, fees or social security or health insurance premiums, or in the event of such a decision or decision - documents confirming payment of these receivables together with possible interest or fines-or a binding agreement regarding the repayment of such receivables; - 6) a statement on the lack of a ruling against him as a preventive measure for not applying for public orders; - 7) declaration of the Contractor about non-execution of a final conviction judgment - Offense on restriction of liberty or a fine within the scope determined by the ordering party based on art. 24 sec. 5 pkt 5 and 6 of the Public Procurement Law,

8) the Contractor's statement that no final administrative decision was issued against him - about violation of obligations under labor law, environment protection law or social security regulations in the scope specified by the ordering party - Based on art. 24 sec. 5 point 7 of the Public Procurement Law,

9) the Contractor's declaration on not paying taxes and fees loreferred to in the Act of January 12, 1991 on taxes and local fees (Journal of Laws of 2016, item 716 with later changes).

11. If the Contractor has his registered office or place of residence outside the territory of the Republic of Poland, instead of the documents referred to in item 10: 10), he shall submit information from the relevant register or, in the absence of such a register, another equivalent tothe document issued by the competent judicial or administrative body of the country in which the Contractor has its registered office or place of residence has the person concerned by the information or document, to the extent specified in art. 24 sec. 1 items 13, 14 and 21 and par. 5 points 5 and 6 of the PPL Act,

12. Documents referred to in point 11 1) and point 11 2) b, should be issued not earlier than 6 months before the deadline for submission of offers. The document referred to in point 11 2) a, should be issued not earlier than 3 months before the expiry of this period.

13.
