Purchase and delivery of an impact crusher and a double-deck classification screening plant for the municipal waste disposal plant in Puławy

Reference Number: ZP / ZUK / 01/2020

1. The subject of the order is the purchase and delivery of: impact crusher and double-deck classifying screen, as part of project No. RPLU.06.03.00-06-0001 / 16 "Adjusting Zagospo betsdonating waste in the Lublin Province to the requirements of RIPOK ", submitted under the RPLU.06.03.00-IZ.00-06-S01 / 2016 call, under Measure 6.3 Waste Management RPO WL for 2014 2020.

Main Site: Place of implementation of orders: Zakład Uniebageliwiania Odpadow Komunalnych, ul. Dęblińska 96, 24-100 Puławy, POLAND.

1) The subject of the order is the purchase and delivery of: impact crusher and double deckad classifying screener, as part of project No. RPLU.06.03.00-06-0001 / 16 "Adaptation of waste management plants in the Lublin Voivodeship to the requirements for RIPOK" submitted under the RPLU.06.03.00-IZ.00-06-S01 / 2016 call, under Measure 6.3 Waste Management RPO WL for the years 2014 2020

2) Technical parameters of the impact crusher:

1 year of production, not older than 2019/2020;

2. self-propelled machine, built on a crawler chassis;

3. processing capacity not less than 60 Mg / h;

4. internal combustion engine, diesel engine with a power of not less than 115 kW, meeting the latest emission standards;

5. 4 hammer crushing system;

6. container capacity not less than 2 m;

7. loading inlet of dimensions not less than 750 x 580 mm;

8. direct drive crushersquint clutch;

9. magnetic separator over the main tape with the option of depositing metal on the right and left;

10. protection system against non-crushing materials;

11. system unlocking the crushing chamber;

12. system enabling remote control of at least the feeding basket and track drive;

13. crusher`s transport weight not more than 28 tons;

14. transport length (pfolding conveyor) not more than 10, 000 mm;

15. transport width not more than 2 500 mm;

16. transport height not more than 3, 200 mm;

17. additional independent (powered by an internal combustion engine) and mobile (on wheels or caterpillars) double-deck screen with a screening area of at least 2.5 m, enabling obtaining product fractions with dimensions 0 31.5 mm; 31.5 63 mm andover 63 mm, with 3 belt conveyors;

18. the screen must have a capacity of not less than a crusher;

19. the screen must come from the manufacturer;

20. screening year, not older than 2019/2020.

Additional parameters:

1. Hybrid solution (min. 20 kVA power generator on at least 1 machine)

2. Possibility to install a rod grate on the upper deck of the screen.

PozosFull requirements:

- Impact crusher and double-deck classification screen must be brand new and cannot be a prototype. Impact crusher and double-deck classifying screen must be prepared for operation and have all required by law certificates and certificates (e.g. such as the European Union CE Certificate) admitting to trading and use in accordance with the requirementsami of the Employer.

3) The subject of the contract also includes all additional services related to the delivery being carried out, in particular commissioning, employee training, the presence of a Contractor`s representative during mechanical, technological and operational start-up tests, as well as all inspections during the warranty period, excluding consumables and materials. natuconsumables. The list of consumables and naturally consumable materials should be attached to the technical and operational documentation for individual devices.

4) A detailed description of the subject of the contract is provided in the ToR, Part III, the description of the subject of the order. does not specify the waydescription of the fulfillment of this condition.

1. The offer should be submitted only in electronic form according to the procedure described in point 24 IDW - SIWZ.

2. The following statements and documents are required confirming the fulfillment of the conditions for participation in the procedure and the absence of grounds for exclusion, i.e. * EAT; * in the case of Contractors jointly applying for the contract, a document establishing the representativeto represent them in the procurement procedure or to represent them in the procedure and conclusion of the public procurement contract:

* appropriate power of attorney - if the authorization to sign the offer does not result directly from the principles of representation disclosed in the relevant register or record;

* commitment of other entities to participate in the implementation of the contract -if applicable.

3. At the request of the Employer, the Contractor shall submit in electronic form:

* a statement on the lack of grounds for exclusion specified in art. 24 paragraph 1 and paragraph 5 points 1 and 8 of the Public Procurement Law, and meeting the conditions for participation in the proceedings;

* list of deliveries made, and in the case of periodic or continuous services also performed;

* information from the National Criminal Register in the scope specified inArt. 24 paragraph 1 item 13, 14 and 21 of the PPL Act;

* certificate from the competent head of the tax office;

* certificate of the appropriate organizational unit of the Social Insurance Institution or Agricultural Social Insurance Fund or other document;

* excerpt from the relevant register or Central Business Register and Information;

* description of technical parameters of the Contractsca transmits without request within 3 days of posting on the website the information referred to in art. 86 section 5 of the Public Procurement Law: statement on belonging / not belonging to the same capital group as referred to in art. 24 paragraph 1 point 23 of the Public Procurement Law

4. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland, it submits:

* information with a replythe relevant register or other equivalent document issued by the competent judicial or administrative authority of the country in which the Contractor has its registered office or place of residence, or the place of residence of the person to whom the information or document relates to the extent specified in art. 24 paragraph 1 item 13, 14, 21 of the Public Procurement Law;

* a document confirming that he is not in arrears with taxes, fees, and insurance premiumsnot social or health or that he has made an agreement with the competent authority on the repayment of these claims together with any interest or fines, in particular he obtained the exemption, deferral or installment of overdue payments in installments or withholding in full the decision of the competent authority;

* document confirming that its liquidation has not been opened or announcedbankruptcy.

If in the country where the Contractor has its registered office or place of residence, the above documents are not issued, they shall be replaced by a document containing the Contractor`s statement. The Employer excludes the Contractor if he has not demonstrated that the conditions for participation in the procedure have been met or has not demonstrated that there are no grounds for exclusion specified in Art. 24 paragraph 1 point 13 23 and para. 5 points 1 and 8 of the Pzp.

5. A detailed description of the statements and documents confirming the fulfillment of the conditions for participation in the proceedings and the absence of grounds for exclusion is presented in point 19 of Part I - IDW - SETC.

6. All information necessary in accordance with art. 13 section 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27.4.2016 on the protection of individuals with regard to the processing of personal dataand on the free movement of such data and the repeal of Directive 95/46 / EC is contained in Annex 7 to the ToR.

7. The Awarding Entity informs that in these proceedings the so-called reverse procedure, referred to in art. 24 aa of the Public Procurement Law
