CPV Description: Microelectronic machinery and apparatus. Wire Bonder the scope of the order includes all the equipment and software necessary to run the complete system, i.e. 3 partss: Main Site: Terms of Delivery: DAP (Incoterms 2010) to the address of delivery and commissioning of the system:

The subject of the order includes the purchase, delivery, assembly and commissioning of a wire bonder for microelectronic devices with full instrumentation hereinafter referred to as "bonder". The scope of the order includes equipment and software necessary to run the complete system, i.e. 3 parts:

1) bonder wire;

2) starter kit enabling bonding (i.e. one of the methods of making electrical connections to devices and semiconductor devices) in ball (bonding) and wedge (wedge bonding) modes;

3) height adjustable table with heating option.

As part of the order, the Contractor shall develop technical documentation covering the complete system.

The Contractor shall provide warranty serviceguarantee for the whole system and will carry out all necessary activities on its behalf for the entire duration of the warranty. The Contractor shall provide a guarantee for a period of 12 months from the date of signing by the Employer of the acceptance report of the subject of the contract without reservation.

The Employer does not impose specific requirements in this respect.

Contractor, in relation to which any of the circumstances referred to in art. 24 paragraph 1 point 12 23 of the PPL Act. The Employer will exclude the Contractor from the procedure for awarding the contract, in relation to which the circumstances referred to in art. 24 paragraph 5 points 1 4 and 8 of the PPL Act. The Contractor is required to submit a current statement as at the date of submission of tendersconfirming that the Contractor:

1) is not subject to exclusion;

2) meets the conditions for participation in the procedure.

2. The declaration referred to in item 1 in the form of a single European order document drawn up in accordance with the template of the standard form set out in the European Commission implementing regulation issued on the basis of art. 59 clause 2 of Directive 2014/24 / EU, called "single dockmentor "or" EAT ", the Contractor is obliged to submit together with the offer in electronic form with a qualified electronic signature, in accordance with the principles set out in chapter X of the ToR. orders, i.e. it has financial resources or creditworthiness in the period not earlymore than 1 month before the deadline for submitting bids in the amount of a minimum of PLN 80, 000.00 (say: eighty thousand Polish zlotys).

b) It is required by the Contractor to show that he is insured against civil liability in the scope of conducted activity related to the subject of the contract for a guarantee sum of at least PLN 80, 000.00 (eighty thousand zlotys)The values provided in the documents confirming the fulfillment of the condition of participation in proceedings in currencies other than PLN, the Contractor, and also when ordering will convert at the average exchange rate of the National Bank of Poland (Table A) as at the date of publication of the contract notice regarding the said procedure in the Official Journal of the European Union.

To confirm the absence of grounds for exclusion from participation in the postProceedings:

a) information from the National Criminal Record in the scope specified in art. 24 paragraph 1 point 13, 14 21 of the Public Procurement Law, issued not earlier than 6 months before the deadline for submitting bids;

b) a certificate from the competent head of the tax office confirming that the Contractor is not in arrears with tax payments, issued not earlier than 3 months before the deadline for submitting bids, oranother document confirming that the Contractor has entered into an agreement with the competent tax authority regarding the repayment of these receivables together with any interest or fines, in particular, he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision of the competent authority;

c) certificate of the appropriate local organizational unitSocial Insurance Institution or Agricultural Social Insurance Fund or other document confirming that the Contractor is not in arrears with payment of social security or health insurance contributions, issued not earlier than 3 months before the deadline for submission of tenders, or other document confirming that the Contractor has concluded an agreement with the competent authority on these repayments duetogether with any interest or fines, in particular, he obtained the exemption, deferral or installment of overdue payments as provided for by law or withholding in full the decision of the competent authority;

d) an excerpt from the relevant register or from the central register and information on business activity, if separate regulations require an entry in the register or records in order to sweatno grounds for exclusion under Article 24 paragraph 5 point 1 of the Public Procurement Law;

e) the Contractor`s statement that he has not issued a final court judgment against him or a final administrative decision on arrears in the payment of taxes, fees or social security or health insurance contributions, or in the case of issuing such a judgment or decision, a document confirming paymentreceivables together with any interest or fines or conclusion of a binding agreement on the repayment of these receivables;

f) statement of the Contractor about the lack of a decision against him as a preventive measure prohibiting him from applying for public contracts;

g) statement of the Contractor on non-payment of taxes and fees local, referred to in the Act of January 12, 1991 on supplylocal taxes and fees (Journal of Laws of 2016, item 716).

Documents of foreign entities. If the Contractor has its registered office or place of residence outside the territory of the Republic of Poland instead of the documents referred to above:

1) in point 2 a), submit information from the relevant register or, in the absence of such register, another equivalent document issued by the competent judicial authority or adminof the country in which the Contractor has its registered office or place of residence or place of residence has the person to whom the information or document relates, to the extent specified in art. 24 paragraph 1 point 13, 14 and 21 of the Public Procurement Law;

2) point 2 b) d) submits a document or documents issued in the country in which the Contractor has its registered office or place of residence, confirming respectively that:

a) there is no payment in arrearstaxes, fees, social security or health insurance contributions or that he has entered into an agreement with the competent authority regarding the repayment of these receivables together with any interest or fines, in particular he obtained the exemption provided for by law, deferment or installment of overdue payments or withholding in full the decision the competent authority;

(b) its liquidation has not been opened orno bankruptcy declared.
